3.3. PROGRAM DESCRIPTION
Dane County Dept of Human Services is seeking a vendor that will provide a mobile response to persons with dementia who are at risk of being hospitalized or removed from their home of choice.
Historical Context:
The Dane County Support Team (DST) was in place from 2009 through May 2018 and implemented by, what was then, South Madison Coalition of the Elderly (SMCE) through a contract with Dane County
Department of Human Services. It was an interdisciplinary team whose focus was helping to relocate persons with dementia who had been emergently detained to an Institute of Mental Disease (IMD), most commonly the Geriatric Treatment Unit at Mendota Mental Health Institute in Madison, because of physically aggressive behaviors that put themselves or others at risk. Additionally, the DST supported persons with dementia who were eligible for long term care funding and appeared to be at risk of an emergency detention due to escalating physical expressions of aggression.
Due to changes in the long term care funding available to Dane County in 2018 the DST was no longer able to be funded after May 2018 and ceased to exist. While the DST ceased to exist, the needs of those they supported – persons with dementia – did not. The need for a dementia mobile response remains an identified need and funding was approved by the Dane County in 2022 to re-create a Dementia
Mobile Support Team.
Program Overview:
https://dcdhs.com/ https://procurement.opengov.com/portal/countyofdane http://www.nlrb.gov/ http://werc.wi.gov/
As part of the Dementia Mobile Support Team, the PROVIDER will provide a mobile, dementia-informed response to persons with dementia expressing challenging behaviors (kicking, hitting, biting, spitting, throwing items, etc) which put them at risk of having to be hospitalized or relocated from their home of choice. When such individuals at risk are referred to the Dementia Mobile Support Team the PROVIDER will be responsible for responding to that referral within 48 hours (excluding holidays and weekends) with an in-person visit to meet the individual. It will be their responsibility to:
• Gather information about the individual including circumstances surrounding their behavioral expressions
• Assess what the individual is attempting to communicate through their expressions and provide training on dementia-informed positive approaches to caregivers (both informal and/or formal) which will help them better understand the needs of the individual and how to respond in a way which minimizes further physical behavioral expressions
• Identify other dementia-informed resources and/or interventions which will allow the individual to stabilize in place whenever possible
• Coordinate with Dementia Support Team geriatric psychiatrist as needed to consult on medication and/or diagnostic assessments
• Create “Life Story” book reflecting the family, interests, preferences and background of the individual with dementia. A “Life Story” book can help a person with dementia to reflect on important information about themselves and strengthen their sense of identity. It can also bring them comfort by reminding them of special memories and other things that are important to them.
• Assist in discharge planning when individuals with dementia are admitted to an Institute of
Mental Disease (IMD) due to their behavioral expressions.
3.4. GOALS
A. Increased capacity to provide a mobile response to assess the needs of an individual with dementia expressing physically challenging behaviors in their home including own home, Community Based Residential Facility, Adult Family Home, etc. throughout Dane County.
B. Caregivers and other residential care providers will be supported by receiving training and guidance specific to the needs of the individual for whom they are caring by the Dementia
Mobile Response Team staff.
C. Individuals with dementia who express behavioral challenges will receive care and support more specific to their needs resulting in them feeling more understood, thereby mitigating the need to express their needs in a physical manner.
D. Persons with dementia will not have to move from their home of choice due to physically challenging behavioral expressions.
E. With the creation of a ‘Life Story” book for individuals with dementia expressing physically challenging behaviors caregivers and residential staff will have an additional tool to use when helping these individuals to stabilize during periods of dysregulation.
F. Person-centered and dementia-informed discharge plans will be created when individuals with dementia return to the community after an IMD stay.
3.5. NEEDS AND EXPECTATIONS

7.8. VENDOR REGISTRATION
Vendors are required to register at the County’s eProcurement Portal. Registration is free and will ensure that you receive notifications of solicitations that match your profile and category code(s).

Once registered, please check to make sure your vendor information including commodity codes is up-to-date by signing into your account at:
https://procurement.opengov.com/portal/countyofdane.

(28) BIDDER RESPONSIBILITY. (a) Any bid, application or proposal for any contract with the county, including public works contracts regulated under chapter 40, shall include a certification indicating whether the bidder has been found by the National Labor Relations Board (NLRB) or the Wisconsin
Employment Relations Committee (WERC) to have violated any statute or regulation regarding labor standards or relations within the last seven years. The purchasing manager shall investigate any such finding and make a recommendation to the committee, which shall determine whether the conduct resulting in the finding affects the bidder’s responsibility to perform the contract.
If you indicate that you have been found by the NLRB or WERC to have such a violation on the Vendor
Information page, you must include a copy of any relevant information regarding such violation with your proposal, bid or application.
Additional information about the NLRB and WERC can be found using the following links:
www.nlrb.gov and http://werc.wi.gov.
7.12. PRELIMINARY EVALUATION
The proposals will first be reviewed to determine if requirements are met. Failure to do so will result in the proposal being rejected. In the event that all vendors do not meet one or more of the requirements, the County reserves the right to continue the evaluation of the proposals and to select the proposal which most closely meets the requirements specified in this RFP.
7.13. PROPOSAL SCORING
Accepted proposals will be reviewed by an evaluation team and scored against the stated criteria. This scoring will determine the ranking of vendors based upon their written proposals.
The cost proposal will be scored using a standard quantitative calculation where the most cost criteria points will be awarded to the proposal with the lowest cost. If the team determines that it is in the best interest of the County to require oral presentations, the highest ranking vendors will be invited to make such presentations. Those vendors that participate in the interview process will then be scored, and the final ranking will be made based upon those scores.
7.14. ORAL PRESENTATIONS/INTERVIEW
Top ranked selected proposers may be required to make oral interview presentations and/or site visits to supplement their proposals, if requested by the County. The County will make every reasonable attempt to schedule each presentation at a time and location that is agreeable to the proposer. Failure of a proposer to conduct a presentation to the County on the date scheduled may result in rejection of the vendor’s proposal.
7.15. RIGHT TO REJECT PROPOSALS & NEGOTIATE CONTRACT TERMS
The County reserves the right to reject any and all proposals and to negotiate the terms of the contract, including the award amount, with the selected proposer prior to entering into a contract. If contract negotiations cannot be concluded successfully with the highest scoring proposer, the County may negotiate a contract with the next highest scoring proposer.
7.16. AWARD & FINAL OFFERS
The award will be granted in one of two ways. The award may be granted to the highest scoring responsive and responsible proposer. Alternatively, the highest scoring proposer or proposers may be requested to submit final and best offers. If final and best offers are requested, they will be evaluated against the stated criteria, scored and ranked. The award will then be granted to the highest scoring proposer.
7.17. NOTIFICATION OF INTENT TO AWARD
As a courtesy, the County may send a notification of award memo to responding vendors at the time of the award.
7.18. GENERAL INSTRUCTIONS ON SUBMITTING COST PROPOSALS
The Cost Proposal section of this RFP is a separate document and can be found on the www.danepurchasing.com as part of the posting for this RFP. The Cost Proposal section shall be completed and submitted separately from the RFP Response document.
The proposal will be scored using a standard quantitative calculation where the most cost criteria points will be awarded to the proposal with the lowest cost.

All prices, costs, and conditions outlined in the proposal shall remain fixed for 180 days.
