About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Human Services (DHS) for the procurement of 10 kitchen countertop suites for the Ann M. Kiley Center located in Waukegan, Illinois. The RFQ specifies that the countertops must be postformed, performance-grade, high-wear laminate with a color and design to be determined based on the vendor's available selection. The RFQ requires a mandatory site visit and bidder's conference on May 21, 2019, at 10 AM, where vendors must confirm the countertop measurements and supplies needed. Vendors must submit their quotes no later than May 24, 2019, at 12:30 PM. The RFQ states that the winning vendor will be selected based on the lowest total bid for all products.
The RFQ provides detailed pricing instructions, including requirements for freight/shipping charges, labor costs, and the total cost of materials and labor. It also includes several terms and conditions, such as the requirement that all quoted prices must be valid for 90 days, no price increases will be accepted, and the agency reserves the right to reject all offers or award the contract in part or in whole. The RFQ notes that DHS may require bidders to submit evidence of their qualifications, and that the question of whether a bidder is responsible may involve an evaluation of their experience, type of facility, expertise, or financial resources.
