About this file

This is a Bidder Response Form for DCR 989A, a state and local contract opportunity issued by the Commonwealth of Massachusetts for animal feed, supplies, and equipment. The form is mandatory for all bidders and requires completion of all sections to avoid disqualification or exclusion from evaluation. Bidders must provide company information including legal name, address, point of contact details, and years in business; identify a contract manager who will serve as the primary point of contact with the Department of Conservation and Resources (DCR); and demonstrate relevant experience by identifying one current or recent service example from within the last five years. The bid was re-posted to meet the Commonwealth's needs and represents a continuation of the original bid number BD-26-1020-DCRCU-DC250-126009.

Pricing must be submitted as a percentage discount off retail pricing for the specified products and services, with the restriction that bidders cannot decrease this discount during the contract term unless unforeseen circumstances warrant a formal change request. All bidders are required to provide evidence of valid general liability insurance coverage or face disqualification, along with submission of supplementary required forms including the Prompt Payment Discount Form, Environmentally Preferable Products/Practices Form, and Business Reference Form. Bidders must affirm under penalty of perjury that they have read and agree to all terms and conditions outlined in the Request for Responses and that all information provided is accurate and truthful.
