About this file
This document is a Required Certifications form (PMT-58) for the Florida Department of Children and Families (DCF) Invitation to Bid (ITB) 2425 055 for Child and Family Well-Being Personnel Training. The document is a mandatory form that potential bidders must complete, detailing organizational information, points of contact, and certifications required to participate in the solicitation. Bidders must provide comprehensive attestations covering areas such as compliance with solicitation requirements, absence of conflicts of interest, no collusion, prohibition of gratuities, vendor list verifications, and scrutinized company compliance. The form includes optional tiebreaking certifications for minority businesses, veteran-owned businesses, Florida businesses, and drug-free workplaces.
The certifications cover critical legal and ethical requirements, including confirmation that the respondent is not listed on various state vendor exclusion lists such as the Convicted Vendors List, Discriminatory Vendors List, and Suspended Vendors List. Respondents must certify they are not engaged in business operations in countries of concern, have not been involved in boycotts, and do not have conflicts that would disqualify them from receiving a contract. The document emphasizes the importance of good faith submissions and warns that any contract could be terminated if gratuities are offered or received, highlighting the state's stringent ethical standards for potential contractors in the child and family well-being training space.
