Pre-Solicitation Feedback Form for DCC LIMS RFP 2025-002

This is a pre-solicitation feedback form designed by the State's Procurement Officer at the Colorado Department of Technology (CDT) to collect vendor questions, concerns, and feedback regarding the Laboratory Information Management System (LIMS) solicitation. The form functions as a structured Excel workbook with two primary sections: a State Questions tab containing 20 questions that vendors must address, and an Additional Vendor Feedback tab allowing vendors to submit supplemental questions or concerns tied to specific solicitation artifacts and sections. The State Questions tab addresses project management and implementation timelines, assumptions and dependencies, requirement concerns, contractor roles and responsibilities, requirement clarity, integration materials, cost estimates, staff qualifications, deliverables and milestones, general concerns, and EVAQ approval status. Vendors are required to provide company name, contact person information, email, and phone number with each submission.

The form requires vendors to evaluate cost-related components including a Rough Order-of-Magnitude Estimate for the proposed solution and feedback on the cost workbook structure (Exhibit C). Vendors must address their ability to comply with staffing qualifications outlined in Exhibit A Section 12 and Attachments 17.1-17.3, as well as provide an independent auditor or CPA attestation per Attachment 13. The Additional Vendor Feedback tab permits vendors to reference 26 specific exhibits and attachments (ranging from Attachment 11 through Attachment 21, and Exhibits A through P) to identify concerns related to particular documentation sections. The form serves as a mechanism to clarify requirements before formal solicitation release and does not contain pricing submission instructions or contract award details.
