About this file
This is a Darfur Contracting Act Certification form required by the State of California for companies bidding on state contracts. The form applies to any company that has conducted business activities or operations outside the United States within the previous three years and seeks to submit a bid or proposal for a California state contract. The Veterans Home of California - Barstow is seeking Temporary Relief Medical Director, Physician, and Physician Assistant Services, with a mandatory walkthrough required as part of the procurement process.
The certification form provides two options for eligible companies: Option 1 requires certification under penalty of perjury that the prospective bidder is not a scrutinized company as defined in Public Contract Code Section 10476, and Option 2 applies to scrutinized companies that have obtained written permission from the Department of General Services to submit a proposal. Companies without international business activities in the previous three years are exempt from completing this certification. The form requires submission of the company name, Federal ID Number, authorized signature, and printed name and title of the signing official. All bidders must comply with these requirements to be considered eligible for award.
