About this file

This is a Darfur Contracting Act Certification form required by the State of California for vendors bidding on state and local contracts. The form applies to the informal competitive solicitation for Sacramento County Courthouse (03-0061 Tani Cantil-Sakauye Sacramento County Courthouse) seeking a Low Temp Combo vending machine. Bidders or proposers with business activities or operations outside the United States within the previous three years must complete the certification to establish eligibility for submission.

The certification requires vendors to select one of three options: certifying they have no international business operations in the past three years, declaring status as a scrutinized company with written Department of General Services permission to bid, or certifying they have international operations but are not a scrutinized company as defined under Public Contract Code section 10476. The form requires completion with company name, Federal ID Number, authorized signature, printed name and title of the signing official, and execution date and location. Vendors selecting Option 3 must provide certification under penalty of perjury that they are duly authorized to bind the proposer or bidder to the stated compliance.
