About this file
This document is a Past Performance Questionnaire for a Request for Quotation (RFQ) 36C25025Q0526 for Homeless Care for Homeless Veterans (HCHV) services at the Aleda E. Lutz VA Medical Center in Saginaw, Michigan. The questionnaire is designed to be completed by previous contract evaluators to assess a potential contractor's performance across multiple criteria, including timely service completion, personnel qualifications, quality control, regulatory compliance, problem-solving abilities, and overall contract performance.
The document provides a detailed evaluation framework with rating options from Outstanding to Unacceptable, covering ten specific performance dimensions such as meeting contractual terms, maintaining qualified personnel, responding to urgent requests, and providing accurate documentation. The Contracting Officer, Angela Higgins, requests completed questionnaires be emailed to angela.freel@va.gov by 15 August 2025, with evaluators encouraged to provide narrative explanations to support their ratings. The ultimate goal is to establish a comprehensive performance history for the potential contractor in the homeless veterans' care service domain.
