About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on federal contracts. The questionnaire must be completed and returned to Patricia Trusler (patricia.trusler@va.gov, 913-946-1115) by April 30, 2026. The form is designed to collect structured feedback from government representatives who have direct experience with a contractor's work.

The questionnaire captures respondent and evaluatee information, contract details (title, number, amount, type, performance period, and place of performance), and a detailed assessment of contractor performance across multiple categories. Performance is rated on a 1-6 scale with descriptors ranging from "Unknown" (1) to "Exceptional" (6), with a "Neutral" option for items not applicable. Evaluation categories include: Quality of Product or Service (four sub-items covering conformance to requirements, accuracy of reports, personnel appropriateness, and subcontractor management); Price Control (cost performance and invoice accuracy); Timeliness/Schedule (task completion and responsiveness); Business Relationships (management effectiveness, communication, presentation quality, notification of issues, and problem-solving cooperation); and Key Personnel (personnel qualifications). Section E requests yes/no responses regarding contract award recommendations, default terminations, cure notices or payment suspensions, and an overall performance rating. All Unsatisfactory, Marginal, Very Good, or Exceptional ratings require supporting comments; questionnaires lacking these comments will be rejected by the government.
