This document contains a reference contract worksheet for offerors responding to a solicitation for nurse staffing services. The solicitation is for the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 22 and seeks references from prior contracts demonstrating experience providing various types of nurse staffing, including numbers of Certified Nurse Assistants, Licensed Vocational Nurses, Registered Nurses, and Nurse Practitioners. For each reference, offerors must provide the name of the contracting entity, contract number, geographic location, dates of performance, average monthly nurse staffing levels by type, whether the reference is for the offeror or proposed subcontractor, and whether the role was as prime contractor or subcontractor. Contact information is also required for a reference from the contracting entity who can verify the information.
