About this file

This document is a Quality Assurance Surveillance Plan (QASP) for a Nursing Home Care contract with the U.S. Department of Veterans Affairs in Oregon. The QASP outlines a systematic method to evaluate contractor performance, with specific performance indicators including timely reporting of veteran hospitalizations, deaths, medical records accessibility, and invoice accuracy. Key government personnel include Contracting Officer Wendy Duval, Contract Specialist Brian Stephen, and multiple Contracting Officer Representatives from VA facilities in Oregon.

The related federal contract opportunity is an indefinite-delivery contract for nursing home services in the Oregon VA Health Care System, with a performance period of up to five years and a total contract ceiling of $45 million. The solicitation will remain open for one year from its posting on SAM.gov, with the minimum contract amount based on one patient for one day at the basic rate for reduced physical function. The VA may issue multiple single-award indefinite-delivery, indefinite-quantity (IDIQ) contracts for the state of Oregon, targeting eligible beneficiaries needing nursing home care.
