About this file

This document is a Procurement File Memorandum from the Arizona Department of Health Services, dated September 8, 2023, authored by Lucas Terry, a Procurement Specialist. The document concerns Contract CTR050592 for the BWCH Health Start Program with Adelante Healthcare, which was originally awarded by the state of Arizona on July 6, 2021, through Request for Proposal BPM002372.

The contract is for a Health Start Program with an initial one-year term and the option to renew annually, not to exceed five years. The Program requested a contract extension on May 17, 2023, and Amendment Three was signed by the supplier on July 6, 2023. Due to a procurement oversight, the amendment was not executed until July 24, 2023. Despite the delay, the state determined that the services are still needed and it is in the best interest of the state to reinstate the contract for the remaining term. The document indicates the contract will continue, with Lucas Terry recommending its continuation and Gina Corwin, the Chief Procurement Officer, having the authority to approve the extension.
