This Request for Proposals (RFP) is issued by the City of Covington, Georgia, for on-call stormwater rehabilitation and maintenance construction services on an annual contract basis. The City seeks qualified contractors to provide complete stormwater work including pipe removal and replacement, drainage structure installation and rehabilitation, roadway and concrete restoration, ditch grading, erosion control, tree removal, and utility conflict resolution. Work will be authorized through individual Task Orders with no guaranteed minimum quantity, dollar value, frequency, or type of work. A nonmandatory pre-bid meeting is scheduled for November 17, 2026, at 10:00 AM at City Hall. The deadline for submitting questions is December 1, 2026, by 3:00 PM, with responses issued by December 15, 2026. Sealed proposals are due by 10:00 AM on January 12, 2027, addressed to Maranna Gilbert at the City of Covington. The initial contract term is one year from the effective date, with the City holding the option to renew for up to two additional one-year periods. At renewal, contractors may request a uniform percentage increase in unit prices, limited to the lesser of four percent or the twelve-month change in the Consumer Price Index for All Urban Consumers.

Contractors must submit complete unit prices for all listed items in the pricing schedule, including separate prices for pipe diameters and cut-depth bands. Unit prices are full compensation for complete installation and include supervision, labor, materials, equipment, delivery, excavation, bedding, backfill, compaction, connections, disposal, testing, cleanup, overhead, and profit. The contract uses a comprehensive unit-price structure with separate pricing for mobilization categories (Standard, Major, and Local Project-to-Project Relocation), traffic control, pipe materials (RCP Class III and HDPE), drainage structures, earthwork, pavement restoration, erosion control, trees, and utility work. Payment for estimated quantities is based on actual work performed and accepted, with Task Order quantities serving as authorization estimates only. The City may include a City-controlled contingency of up to fifteen percent of the base Task Order amount for authorized additional or changed work. Contractors must maintain emergency response availability twenty-four hours per day, seven days per week, with a qualified representative on-site within two hours of notification. All proposals require evidence of successful completion of at least three comparable projects, identification of key personnel and equipment, three owner references, proof of required insurance and licenses, and completed affidavits including E-Verify compliance and conflict-of-interest disclosures. Evaluation of responsive proposals will use a 100-point matrix considering relevant company experience and past performance (25 points), key personnel, crews, equipment and capacity (20 points), organization, management, and safety (10 points), project approach and emergency response (20 points), and price (25 points).

2. Contract Purpose and Term

2.1 Purpose

The resulting contract will establish complete unit pricing for recurring stormwater rehabilitation, maintenance, and associated civil work at locations identified by the City. Work shall be limited to projects with a defined scope authorized by the City. Each Task Order shall identify the project location, work, applicable prices, schedule, and

Maximum Authorized Amount. A signed Task Order is required before work begins, except for emergency work authorized under Section 2.1.1.

2.1.1 Emergency Availability and Response

The Contractor shall maintain emergency response availability twenty-four (24) hours per day, seven (7) days per week, including weekends and holidays, and provide primary and backup emergency contacts.

Verbal authorization from the City's designated representative is sufficient to begin emergency work. The

Contractor shall immediately begin mobilizing upon the City's notification and authorization and shall not delay response pending a signed Task Order or other written documentation. The City shall confirm the authorization in writing as soon as practicable.

A qualified Contractor representative with authority to direct the work shall be on site within two (2) hours of the

City's initial notification and authorization. The Contractor shall immediately initiate protective and stabilization work upon arrival and furnish personnel, equipment, and materials necessary to meet the emergency response requirements.

Within six (6) hours of the City's initial notification and authorization, the Contractor shall stabilize the affected area and implement temporary measures necessary to control immediate hazards to the public, property, and infrastructure. Measures may include barricades, traffic control, securing excavations, temporary drainage, and other measures appropriate to the emergency. The Contractor shall continue emergency operations until the City determines that immediate hazards have been controlled and the site is sufficiently stabilized. Permanent repairs and restoration shall proceed as directed by the City.

For an emergency, the City may verbally establish the initial scope necessary to respond and an initial authorized expenditure limit. The scope and limit shall be documented in the subsequent written authorization and may be supplemented as conditions are identified and additional work is authorized. The Contractor shall promptly report changing conditions and anticipated costs and seek City authorization before expanding the work beyond the authorized response, subject to the immediate-protection provision in Section 4.4.

2.1.2 Routine Response and Commencement

The Contractor shall acknowledge receipt of the City's request for non-emergency services within one (1) business day and, when requested, attend a site visit within five (5) business days of the original request, unless otherwise agreed by the City. Estimate deadlines shall be coordinated with the City.

Unless otherwise specified in the Task Order or approved in writing by the City, the Contractor shall commence on-site work within ten (10) calendar days after receipt of the City's written Notice to Proceed and diligently pursue the work toward the estimated completion date shown in the Task Order. The Contractor shall promptly notify the City of anticipated delays. Revisions to the estimated completion date shall be coordinated with the City. Ordering materials or submitting paperwork alone does not constitute commencement of on-site work. Emergency work remains subject to Section 2.1.1.

2.2 Term and Renewal

The initial term will be one year from the effective date. The City may renew the contract for up to two additional one-year periods by written agreement. At renewal, the Contractor may request one uniform percentage increase

September 29, 2026 | RFP # 2026-26 | Page 14 in contract unit prices. No adjustment is automatic. The City may approve, negotiate, or reject the request;

individual item increases are not permitted.

The maximum permitted increase is the lesser of four percent (4%) or the positive twelve-month percentage change in the Consumer Price Index for All Urban Consumers (CPI-U), All Items, U.S. City Average, not seasonally adjusted, published by the U.S. Bureau of Labor Statistics. Use the latest monthly index published and available sixty (60) calendar days before the renewal effective date and compare it with the index for the same month one year earlier. Calculate the percentage change as 100 multiplied by (current index divided by prior-year index minus 1). The Contractor shall provide the index values and calculation. If that change is zero or negative, no increase is permitted and existing prices remain unchanged. Any approved uniform increase shall be included in the written renewal agreement and applied to the immediately preceding contract-year unit prices. No other escalation applies unless authorized by formal amendment.

3. Scope of Services The Contractor shall furnish supervision, labor, materials, equipment, transportation, permits assigned to the

Contractor, traffic control, erosion control, testing, cleanup, and incidentals necessary to complete each authorized

Task Order. Typical work may include:

• Open-cut removal and replacement of storm drainage pipe; installation of new storm pipe; and removal of existing pipe without replacement when directed by the City.

• Removal, replacement, construction, adjustment, or rehabilitation of catch basins, curb inlets, junction boxes, drainage access structures, headwalls with wingwalls, and related appurtenances, including removal of existing drainage structures without replacement.

• Roadway, curb and gutter, sidewalk, concrete driveway, asphalt driveway, shoulder, and landscaped-area restoration.

• Ditch and channel grading to establish positive drainage using the minimum excavation reasonably necessary.

• Riprap, geotextile, sod, seeding, erosion-control measures, and final site stabilization.

• Excavation, rock excavation, City-authorized imported suitable fill, authorized unsuitable-soil removal, dewatering when specifically authorized, and disposal that is not already included in another complete-pay item.

• Traffic control, public protection, utility coordination, cleanup, and restoration.

• Tree removal, small-area clearing and grubbing, and fence or gravel-surface restoration associated with authorized stormwater work.

• Water and sanitary sewer adjustments, relocations, and associated listed services necessary to resolve conflicts with authorized stormwater work, when expressly included in the Task Order and coordinated with the

City's utility representatives.

Storm-drain cleaning, CCTV inspection, cured-in-place pipe, sliplining, geopolymer lining, and other specialized stormwater trenchless rehabilitation are outside the base pricing schedule unless added by addendum or authorized as unlisted work. The specific sanitary sewer cleaning, CCTV, and manhole rehabilitation items in

Attachment A remain available for authorized utility-conflict work. Routine maintenance of the Contractor's erosion controls remains included in the applicable work.

4. Task-Order Administration

4.1 Development and Authorization

1. The City's designated Engineering Department representative may identify proposed Work, develop the scope, coordinate a site review with the Contractor, review proposed quantities and applicable contract unit prices, and prepare the Task Order.

2. The Contractor will submit an itemized estimate using applicable contract unit prices and reasonable quantities. Any proposed unlisted work must be separately identified and supported.

3. The City will prepare the Task Order, including scope, estimated quantities, schedule, contingency, payment terms, and any special requirements.

4. Except for emergency work under Section 2.1.1, a Task Order does not authorize Work or obligate the City until it has been signed by authorized representatives of the City and Contractor and any required purchase order, notice to proceed, or equivalent written authorization has been issued. Emergency verbal authorization and subsequent written confirmation shall identify the scope and authorized limits as required by Section 2.1.1.

September 29, 2026 | RFP # 2026-26 | Page 15

4.2 Estimated Quantities and Final Payment

Task-Order quantities are estimates for authorization and budgeting. Payment will be based on the applicable contract prices and work actually performed and accepted, subject to the Task Order. The Contractor shall maintain records sufficient to support invoiced quantities and shall provide measurements, tickets, invoices, or other documentation when requested by the City. Routine work is not required to stop solely for photographic documentation unless the Task Order expressly requires it.

4.3 City-Controlled Contingency

Each Task Order may include a City-controlled contingency of up to fifteen percent (15%) of the base Task Order amount. The City shall select the percentage or amount according to the project's risks and uncertainties.

Contingency is not guaranteed compensation and may be used only for additional or changed work authorized by the City's designated representative, including verbal emergency authorization under Section 2.1.1. The Contractor shall provide supporting documentation when requested. Unused contingency will not be paid. Base amount plus authorized contingency establishes the Maximum Authorized Amount shown in the Task Order.

4.4 Work Without Authorization

The Contractor assumes the risk of nonpayment for work beyond the authorized scope or amount without prior City authorization in the form required by this contract. Verbal emergency authorization under Section 2.1.1 is sufficient to begin the authorized emergency response and shall be confirmed in writing. Immediate actions reasonably necessary to protect life or property shall be promptly reported to the City.

Minor rehabilitation may be included in the original Task Order when visible conditions permit the City to identify the repair in advance. If an existing drainage structure designated for removal and replacement is found after exposure to be suitable for minor rehabilitation, the Contractor shall notify the City before proceeding. When substituting minor rehabilitation for replacement will not increase the authorized Task Order amount, the City's designated representative may verbally authorize the substitution. The Contractor shall identify the original replacement item and credit, the rehabilitation item actually performed, and the resulting net reduction on the final invoice. A written Task Order amendment or other written change authorization is required before proceeding with a substitution or changed condition that would increase the authorized Task Order amount.

The City's designated representative may verbally direct a reduction or deletion of authorized Work when the direction will not increase the Task Order amount. The reduction shall be documented through final measured quantities, applicable credits, and the Contractor's final invoice. Verbal direction does not authorize additional compensation or an increase in the maximum authorized amount.

5. Pricing and Payment Rules

5.1 Complete Unit Prices

Unless a pricing item expressly states otherwise, each unit price is full compensation for a complete and accepted installation. Prices include supervision, labor, materials, equipment, delivery, ordinary excavation, trench preparation, bedding, backfill, compaction, cutting, connections, removal and lawful disposal of replaced materials, ordinary excavated spoil displaced by the completed work, testing, cleanup, overhead, profit, and incidentals.

All ordinary excavation, bedding aggregate, foundation aggregate, graded aggregate base, crusher run, inlet protection, and related materials necessary to complete a priced unit of work are included in the applicable unit price. Listing an included operation or material separately in a Task Order does not make it separately payable.

Separate payment is permitted only for City-authorized, distinct additional work outside the scope of the applicable unit-price item.

5.2 Remove-and-Replace Pipe

Remove-and-replace pipe items include locating and exposing the existing pipe, removal, loading, transportation and lawful disposal of the existing pipe, ordinary excavation and spoil, furnishing and installing new pipe, bedding, backfill, compaction, connections, cleanup, and restoration preparation. Existing-pipe removal, pipe debris, or ordinary displaced trench spoil shall not be billed again under another item.

September 29, 2026 | RFP # 2026-26 | Page 16

5.3 New Pipe Construction

New-construction pipe items apply only where the City directs installation of pipe where no pipe previously existed.

They include complete installation but do not include removal of existing pipe. The City expects most contract work to be priced under the remove-and-replace section.

5.3.1 Pipe Removal Without Replacement

Removal-only items are measured by LF of existing pipe actually removed, by the diameter and cut-depth band stated in Attachment A. They include ordinary excavation, removal, loading, lawful disposal, backfill, compaction, and restoration preparation. Surface restoration is paid under the applicable authorized restoration item. These items are not payable when removal is included in a remove-and-replace pipe item.

5.3.2 Pipe Cut Depth and Measurement

Trench depth shall be measured vertically at the pipe centerline from the existing ground or pavement surface before excavation to the required bottom of the trench, including bedding depth. Unauthorized over-excavation shall not increase the payable depth. Measure installed pipe length along the completed pipe centerline. For removal-only work, measure the actual length of pipe removed. Allocate length to each applicable depth band where depth changes. Depth bands are mutually exclusive: up to and including 8 feet; over 8 through 12 feet; and over 12 through 16 feet. Work requiring trench depths greater than 16 feet requires prior City-approved unlisted pricing. A pipe segment is paid under one depth band only. The listed depth range does not establish allowable cover or structural capacity; the Task Order and applicable technical requirements control suitability for the actual loading and cover.

5.4 Unsuitable Soil and Waste

Unsuitable-soil removal is payable only when the City specifically authorizes removal of material that cannot remain or be reused because of field conditions. The item does not include existing pipe, concrete, asphalt, vegetation, demolition debris, ordinary excavation, or spoil displaced by bedding, aggregate, structures, or other completed work when disposal is included in the applicable pay item. Materials shall be kept segregated or measured by a method acceptable to the City. No material may be billed simultaneously as unsuitable soil and as another removal or disposal item.

City-authorized imported suitable fill is payable only when the City expressly authorizes imported fill beyond material included in another complete unit-price item. Payment is based on the authorized compacted in-place cubic-yard volume. The item is not separately payable for ordinary pipe bedding, embedment, trench backfill, aggregate base, drainage-structure foundation material, or material required because of the Contractor's excavation methods, over-excavation, material handling, sequencing, or failure to protect otherwise suitable material.

5.5 Approved Unlisted Work

Work not covered by a listed item requires a City-approved scope and a mutually agreed complete unit price or lump-sum price before performance, subject to the emergency authorization provisions in Section 2.1.1. The proposal shall identify labor, equipment, materials, subcontractors, assumptions, and exclusions sufficiently for the

City to evaluate the complete price. No automatic material or subcontractor percentage markup or open-ended subcontracted-work allowance applies. The Contractor remains responsible for subcontracted work under

Attachment D. Unlisted-work prices shall not duplicate amounts included in listed items.

5.6 Unlisted Work Performed by the Contractor's Own Crews

This section applies to unlisted work performed by the Contractor's employees and equipment. The City must approve the scope and a complete unit price or lump-sum price before work begins, except that verbal authorization is sufficient for emergency work under Section 2.1.1 and shall be confirmed in writing. Time-and-material payment is not permitted. Unlisted work shall be paid only at an agreed complete unit price or lump-sum price. Listed work remains payable at the applicable contract unit price; no separate payment is due for labor, materials, or equipment included in that price.

September 29, 2026 | RFP # 2026-26 | Page 17

5.7 Invoicing, Partial Payments, and Retainage

Unless a Task Order provides otherwise, the Contractor shall submit one invoice after completion and acceptance of the work. When partial payments are authorized, the Task Order shall state the payment schedule. Retainage of five percent (5%) shall be withheld from every partial payment until final completion and acceptance. The City may request reasonable supporting documentation when an invoiced amount or quantity requires clarification.

5.8 Measurement, Duplicate Payment, and Conditional Items

EA is measured by each complete, accepted occurrence; LF along the installed or completed centerline, except that removal-only pipe work is measured by the actual length of pipe removed; VF only as expressly defined for structure depth or adjustment; SY by completed surface area; CY by the applicable authorized excavation or compacted in-place volume; TON by certified weight ticket; and DAY by each City-authorized day the specified operation is actually provided. Pipe LF prices use the cut-depth bands defined in Section 5.3.2. Estimated Task

Order quantities are payment estimates, not guaranteed quantities. Final payment is based on completed quantities under the applicable unit and Task Order limits.

The Contractor shall not receive payment under more than one item for the same labor, equipment, material, excavation, hauling, disposal, mobilization, restoration, or incidental operation. When an activity is included in a complete unit-price item, no separate payment is due unless the Task Order expressly identifies and authorizes a distinct additional scope. Conditional items, including Major Mobilization, unsuitable-soil disposal, trench rock, Type C silt fence, and unlisted work, require City authorization before separate payment except when immediate action is necessary to protect life or property and prompt written notice is provided.

6. Mobilization and Traffic Control

6.1 Mobilization Categories

Mobilization will not be paid as a percentage of a Task Order. The City will select the applicable category in the

Task Order:

• Standard Mobilization - deployment of the crew and ordinary equipment reasonably required for a typical single-site Task Order.

• Major Mobilization - deployment requiring substantial specialized or heavy equipment, multiple hauling operations, or logistics materially beyond a standard mobilization, as expressly identified in the Task Order.

• Local Project-to-Project Relocation - direct relocation of an established crew and equipment from one nearby

City Task Order site to another without return to the Contractor's yard or a new full deployment.

Only one mobilization category applies to a particular deployment unless the City expressly authorizes otherwise.

Repeated daily travel, ordinary delivery, internal movement within a project, and remobilization caused by the

Contractor's sequencing, staffing, or correction of deficient work are incidental.

6.2 Traffic Control

The Contractor shall maintain safe access and traffic operations consistent with the Manual on Uniform Traffic

Control Devices, applicable Georgia requirements, and the Task Order. Traffic-control items are payable only when authorized. A-5 is paid per authorized workday, including the two flaggers and listed traffic-control equipment. Any partial workday counts as one day. A-4 is not separately payable for setup already included in A-5.

Ordinary cones, signs, barricades, flagging coordination, and maintenance of the work zone necessary for a listed traffic-control item are included in that item. Existing pavement markings disturbed by the work shall be restored in kind as incidental work unless the Task Order states otherwise.

7. Technical Requirements

7.1 General Standard

Work shall be performed in a good and workmanlike manner, true to line and grade, with positive drainage and complete connections. The Contractor is responsible for verifying existing conditions, dimensions, pipe inverts, utilities, grades, access, and quantities necessary to complete the Task Order. The City may observe and inspect work at any time. Routine construction stages are not automatic hold points unless the Task Order expressly establishes one, and the Contractor may continue ordinary Work without waiting for routine City inspection.

September 29, 2026 | RFP # 2026-26 | Page 18

All work shall comply with all applicable Georgia Department of Transportation (GDOT) standards, specifications, and construction details, except where the contract or applicable Task Order expressly establishes a different requirement. Water and sewer work shall also comply with the City's applicable utility standards and authorized utility details. Conflicts shall be referred to the City for written direction before the affected work proceeds, subject to emergency authorization under Section 2.1.1.

If an unforeseen condition may materially change the authorized scope, cost, quantity, alignment, elevation, construction method, property impact, utility conflict, or safety requirements, the Contractor shall stop only the affected portion, promptly notify the City, and protect the site. Unaffected Work may continue safely without prejudicing the City's decision. Changes increasing the authorized amount require written City direction, except for authorized emergency work under Section 2.1.1. A rehabilitation-for-replacement substitution that does not increase the authorized amount may proceed upon verbal authorization under Section 4.4. Work beyond the authorized scope without the authorization required by this contract is not entitled to additional payment, subject to the immediate-protection provision in Section 4.4.

7.2 Pipe Installation

• Separate pricing is provided for Class III reinforced concrete pipe (RCP) and corrugated high-density polyethylene (HDPE). The City shall identify the pipe material, diameter, joint requirements, and applicable installation requirements in the Task Order. Listed RCP sizes extend through 60 inches and listed HDPE sizes extend through 48 inches. Material substitutions require City approval.

RCP shall comply with ASTM C76, Class III, and applicable GDOT requirements. HDPE shall comply with applicable GDOT material and installation requirements. Pipe selection and installation shall be suitable for the actual loading and cover. Requirements outside the listed pricing basis shall be addressed through the contract change or approved unlisted-work provisions.

• Pipe joints and connections shall be compatible with the specified material and service and installed and sealed according to the manufacturer's written instructions and the applicable specifications. Mortar may be used only where compatible with the pipe and approved method; it is not a universal joint treatment for plastic or metal pipe.

• Provide bedding in accordance with the manufacturer's instructions and applicable standards, but not less than six inches below RCP. Place and compact suitable bedding and embedment through the spring line.

• Maintain required cover, line, grade, and positive flow. Field conditions or inadequate cover may require a different pipe class, trench treatment, or written change authorization.

• Connections to existing structures and pipes are included in the applicable complete pipe or structure item.

Separate payment is permitted only for City-authorized, distinct additional connection work outside that item’s scope. The component-payment rules for water and sanitary sewer work in Section 7.7 remain applicable.

7.3 Structures

Drainage structures shall be sized and configured for connected pipes and field conditions. Square or rectangular structures shall have minimum clear internal dimensions of 3 feet by 3 feet unless site conditions dictate otherwise and the City authorizes an exception. Round precast structures are separately identified by 4-, 5-, and 6-foot inside diameter; square precast structures are identified by 4-by-4-, 5-by-5-, and 6-by-6-foot clear inside dimensions. A listed size does not establish that the structure accommodates every connected pipe or loading condition.

The City shall select or approve masonry, precast, or cast-in-place structure construction through the Task Order, using the applicable listed item or an approved unlisted-work price. D-1 and D-2 cover built-in-place curb inlets under the applicable GDOT 1033D or 1034D detail; either configuration is included in the listed price. D-4 and D-5 cover built-in-place grated area inlets. Precast curb inlets, grated area inlets, and catch basins of the listed P-series configurations shall be paid under the corresponding P-series item. D-7 and D-8 cover drainage access or junction manholes with manhole rings and covers, not P-series catch basins. Other general structure items permit the construction methods stated in their descriptions. No structure shall be paid under both a general item and a P-series item. The Task Order shall identify the applicable standard detail, configuration, dimensions, openings, loading, and construction method. Assemblies substantially outside a listed item’s scope require an approved unlisted price. Each structure shall be properly founded, connected, formed for positive drainage, adjusted to final grade, and equipped with the hardware required by its complete-structure item. The headwall items in Section J are precast only.

September 29, 2026 | RFP # 2026-26 | Page 19

Complete new and remove-and-replace structure items include excavation, removal and lawful disposal where applicable, foundation preparation, bedding, backfill, compaction, pipe openings and connections, formed invert, adjustment to final grade, specified top hardware, and cleanup. Structure depth is measured from finished rim or grate elevation to the lowest interior invert. The EA price includes depth through three feet. For structures exceeding three feet, subtract the three-foot base depth from the measured structure depth and round the additional depth to the nearest whole vertical foot, with a minimum payment quantity of one vertical foot.

Adjustment to grade is not separately payable when included in a complete new or replacement structure item.

Minor rehabilitation of a retained drainage structure is measured by each structure and includes localized masonry rebuilding, repointing, patching, filling voids, replacement of isolated damaged brick or block, sealing around pipe penetrations, and localized repair of the flow channel and benching. It includes preparation, disposal of repair-generated material, masonry, mortar or concrete, forming, finishing, curing, and incidental cleaning. Major reconstruction requires the applicable replacement item or approved unlisted work. Hardware, top replacement, and adjustment are separately paid only when not included in another authorized complete item.

Replacement tops are measured by EA for the configuration and supporting structure size listed in the D-17 series.

Each price includes removal and disposal of the old top, forming or precast fabrication, reinforcement, concrete, bearing surfaces, curing, the required opening, and connection to the retained structure. Each price includes furnishing and installing the complete top assembly with new compatible rings and covers, frames and grates, hoods, and other hardware required by the applicable configuration. The supporting structure below the top is retained. A single-wing curb-inlet top may be left- or right-hand, as directed by the City; the same EA price applies to either orientation. The Task Order shall identify the applicable detail, dimensions, loading, opening, and seating requirements. Custom tops outside the listed configurations require an approved unlisted price. Authorized adjustment of a retained structure to grade is measured by the actual amount of raising or lowering, rounded to the nearest whole vertical foot, with a minimum payment quantity of one vertical foot. Adjustment is not separately payable when included in a complete replacement item.

Each separately priced hardware item includes removal and disposal of the corresponding existing component, furnishing and installing a compatible replacement, fastening and adjustment materials, and mortar or concrete immediately necessary for seating. Separate hardware items apply when only the hardware is replaced and the existing top is retained. No separate hardware payment is permitted for components included in a D-17 complete top assembly. The same removal, seating, or installation operation shall not be paid twice. No separate hardware payment is made for components already included in a complete new or replacement structure item. Substantial wall or top reconstruction is separately authorized under the applicable item.

The minor nonstructural concrete pipe-end treatment or collar item includes minor hand-formed concrete around an exposed pipe inlet or outlet for stabilization, closure, erosion resistance, or transition into adjoining grade. It excludes structural headwalls and wingwalls. Precast headwalls with wingwalls are separately priced by EA and connecting pipe diameter in Attachment A, through 60-inch pipe. Each complete assembly includes the precast headwall, both wingwalls, any apron required by the applicable standard detail, furnishing, delivery, excavation, foundation preparation, installation, connections, backfill, compaction, and cleanup. The Task Order shall identify the applicable standard detail and pipe connection requirements. No overlapping collar payment is allowed. Work outside the listed assembly scope requires approved unlisted pricing.

Removal of an existing drainage structure without replacement is paid under D-19 and includes demolition, removal, disposal, backfill, and compaction. Surface restoration is measured under the applicable restoration item.

D-19 is not payable when removal is already included in another complete item. The Task Order shall identify retained pipe connections and any separately authorized pipe abandonment or closure work.

7.4 Roadway and Concrete Restoration

Restore pavement, curb, sidewalk, and driveway areas to the material, thickness, grade, jointing, and limits stated in the Task Order and applicable City detail. Concrete driveway restoration shall use the thickness stated in the applicable pay item: 6 inches for residential driveways and 8 inches for commercial driveways. Match existing grades, alignment, and finish unless directed otherwise. Different concrete thicknesses require approved pricing before placement. Asphalt driveway restoration shall match the existing driveway type and configuration unless directed otherwise. Saw cutting, edge preparation, temporary protection, curing, and restoration of disturbed markings are incidental to the applicable restoration item.

For full-depth concrete roadway restoration, trench excavation within the roadway shall be limited to the width reasonably necessary to perform the authorized pipe or structure work and should not ordinarily exceed four feet unless field conditions, pipe size, utility conflicts, safe excavation requirements, or the Contractor's approved means and methods require a greater width. Full-depth concrete roadway restoration shall extend approximately one foot beyond each side of the actual trench excavation to form the pavement bridge-cap area, unless different limits are identified in the Task Order or directed by the City. See Standard Detail 1 - Full-Depth Concrete

Roadway Restoration.

Unless the applicable Task Order expressly states otherwise, roadway trench restoration shall consist entirely of concrete placed to the required pavement-section depth and finished flush with the adjoining roadway. Asphalt pavement or asphalt topping shall be used only when expressly required by the City in the applicable Task Order.

This requirement applies to the roadway pavement section above properly compacted trench backfill; it does not require the entire excavation to be filled with concrete. Payment shall be based on the authorized volume of concrete installed under the applicable CY item. Excavation or pavement removal made wider solely for the

Contractor's convenience shall not increase the payable restoration quantity.

7.5 Ditch Grading, Riprap, and Stabilization

Grade ditches and channels only as necessary to establish positive drainage and stable transitions, removing as little material as reasonably possible. Type A silt fence (Sd1-NS) shall be used for ordinary non-sensitive applications unless the Task Order or applicable erosion-control requirements call for Type C silt fence (Sd1-S) or another sediment barrier. Wire backing is not required for Type A silt fence. Type III riprap shall be installed over suitable geotextile unless the Task Order states otherwise. Bermuda sod shall be used where sod is specified, unless the City directs another matching turf. Final stabilization includes preparation, watering, establishment, and repair of failed areas during the applicable warranty period.

7.5.1 Trees and Small-Area Clearing

Tree-removal prices are per tree measured by trunk diameter at breast height, 4.5 feet above ground. To avoid gaps, the 6–12-inch category includes diameters at least 6 but less than 13 inches; the 13–23-inch category includes at least 13 but less than 24 inches; and the 24-inch-and-larger category includes at least 24 inches. Each tree price includes cutting, stump and root removal, lawful disposal, backfill, compaction, and restoration of the tree-removal area. No separate payment is made for those included operations.

Small-area clearing and grubbing is measured by SY within City-authorized limits and includes brush, vegetation, and root removal, disposal, and rough grading. Trees paid under separate tree items, City-authorized imported fill, and final stabilization are excluded. Quantities and invoices shall prevent duplicate payment for clearing work included in another complete item.

7.5.2 Fence and Gravel Restoration

Fence removal and replacement is measured by LF for chain-link or wood privacy fencing matching the existing height and construction. Each price includes removal, disposal, new posts and foundations, fabric or panels, rails and ordinary fittings, installation, and cleanup. The Task Order shall identify the replacement limits and any separately authorized gate work. Gravel surface restoration is measured by SY and shall match the existing gravel material and compacted thickness. The price includes preparation, supplying aggregate, placement, grading, and compaction. Gravel surface restoration is not a second payment for bedding or base already included in another item.

7.6 Utilities, Safety, and Environmental Controls

The Contractor shall comply with utility-location laws, protect existing utilities and improvements, maintain a safe work site, and comply with applicable erosion, sediment, environmental, and occupational-safety requirements.

Means and methods, trench safety, protective systems, dewatering, and site security are the Contractor's responsibility. Any separately payable extraordinary measure must be identified in the Task Order or authorized in writing.

7.7 Water and Sanitary Sewer Conflict Work

Water and sanitary sewer work is limited to work expressly authorized in a Task Order to address stormwater-related utility conflicts and associated restoration. The Contractor shall coordinate shutdowns, service interruptions, connections, testing, and return to service with the City's utility representatives and shall not operate utility valves or interrupt service without authorization.

Sections K and L of Attachment A provide prices for authorized water and sanitary sewer work. Prices include the labor, materials, equipment, and incidentals required for each described pay item.

For utility pipe items, separately listed fittings, hydrant assemblies, thrust collars, encasement, sleeve connections, and testing are separately payable only when specifically identified in the Task Order and excluded from the applicable pipe price. The same component or operation may not be paid twice. Service drilling is the installation operation; the Task Order shall identify the associated pipe or casing item. Sewer manhole base, riser, and hardware items are components of an assembly, not multiple complete-manhole payments. Boot connections that include a new invert exclude separate payment for the same invert. Required product classes, depths, component dimensions, coatings, test procedures, and limits shall be identified in the Task Order in accordance with applicable utility standards. This specific measurement rule controls over the general complete-unit-price inclusions where necessary to preserve the listed utility components.

8. Proposal Requirements and Evaluation

8.1 Required Proposal Contents

• Signed proposal and acknowledgment of all addenda.

• Company profile, legal name, address, and authorized representative.

• Evidence of successful completion of at least three projects comparable in scope and complexity to the anticipated work within the preceding five years. Identify the work, proposer's role, completion date, and owner reference for each project; comparable public or private work may be submitted.

• Identify the proposed supervisor and key personnel, their relevant qualifications, available crews and equipment, and primary and backup emergency contacts. Demonstrate the ability to meet the routine and emergency response requirements and the proposed Task Order delivery approach.

• At least three owner references for the comparable projects identified above, including current contact information.

• Completed Unit-Price Schedule with a unit price for every listed item.

• Required affidavits, licenses, insurance information, and disclosure forms.

• Describe the approach to estimating, scheduling, communication, utility coordination, traffic control, safety, emergency response, and maintaining sufficient crews and equipment for authorized work.

8.2 Evaluation

Responsive proposals meeting the required minimum qualifications will be evaluated using the following 100-point matrix. The City will consider the quality and relevance of the submitted evidence within each category.

Criterion Points

Relevant company experience and past performance 25

Key personnel, available crews, equipment, and capacity 20

Organization, management, and safety 10

Project approach and emergency response 20

Price 25

Total 100

Before proposals are due, the City shall publish a representative pricing worksheet using the same quantities and assumptions for every proposer. The issued worksheet shall state the 25-point scoring method, included items, quantity weights, and treatment of unbalanced prices. Price evaluation shall use complete pricing schedules

September 29, 2026 | RFP # 2026-26 | Page 112 meeting Section 8.1 and Attachment A. Quantities are for evaluation only and guarantee no workload. The City may examine individual prices and overall reasonableness.

8.3 Award

Award, if made, will be to the responsible proposer whose proposal is determined to provide the best value to the

City under the stated criteria. The City may verify references, financial responsibility, licensing, experience, equipment, staffing, and ability to perform. The contract does not create an exclusive right to City work.

9. Contract Conditions

9.1 Insurance

The selected Contractor shall maintain the insurance stated in Attachment D throughout the contract and each

Task Order and shall furnish certificates and required endorsements acceptable to the City before work begins.

9.2 Bonds

Performance and payment bonds shall be furnished when required by Georgia law and may be required when expressly stated in a Task Order. No automatic bond percentage or bond fee will be paid. If a Task Order requires a task-specific bond and the Contractor incurs an incremental surety premium, the City may reimburse only the actual documented incremental premium supported by the surety or agent. No markup, overhead, profit, or administrative percentage applies, and no payment is due without documentation of the incremental premium.

9.3 Compliance, Indemnity, and Responsibility

The Contractor shall comply with applicable federal, state, and local laws; licensing requirements; E-Verify requirements; nondiscrimination obligations; taxes; permits assigned to the Contractor; and safety requirements.

Indemnification, sovereign-immunity, termination, records, audit, venue, and related terms are stated in Attachment

D.

9.4 Warranty and Correction

The Contractor warrants for one year after acceptance, or for a longer manufacturer warranty expressly incorporated into a Task Order, that work will be new unless otherwise specified, of good quality, free from defects, and consistent with the Task Order. The Contractor shall promptly correct defective or nonconforming work at no additional cost.

9.5 Termination and Suspension

The City may suspend work, terminate a Task Order, or terminate the contract for cause or convenience under the final contract terms. The Contractor shall protect completed work, stop incurring avoidable costs, and provide records necessary to determine payment for authorized and accepted work.
