Text version
This letter is to inform you that per Section V of Illinois Executive Order 2021-20 (COVID-19 Executive Order No.87) (Executive Order (illinois.gov)), issued by Governor JB Pritzker on August 26, 2021, all individuals employed by, volunteering for, or contracted to provide services for a Health Care Facility, or employed by an entity that is contracted to provide services to a Health Care Facility are required to be vaccinated to work within the State Operated Developmental Centers and State Operated Mental Health Centers. All contractors and vendors who work at these centers are required to show proof of vaccination for both doses of a 2-dose COVID-19 vaccine series or a single dose COVID-19 vaccine by no later than October 4, 2021.
Note: This does not include any person who is present at a State Operated Developmental Centers and State Operated Mental Health Centers for only a short period of time and whose moments of close physical proximity to others on site are fleeting, as determined by the facility (e.g., contractors making deliveries to a site where they remain physically distanced from others or briefly enter a site to pick up a shipment). However, you are receiving this because we have determined that you do not meet that exception.
Pursuant to the order, proof of vaccination status may be met by providing one of the following: (1) a CDC COVID-19 Vaccination record card or photograph of the card; (2) documentation of vaccination from a healthcare provider or electronic health record; or (3) state immunization records (https://idphportal.illinois.gov/s/?language=en_US).
Individuals are exempt from the requirements to be fully vaccinate against COVID-19 if (1) vaccination is medically contraindicated, or (2) vaccination would require the individual to violate or forgo a sincerely held religious belief, practice, or observance. Individuals who demonstrate they meet the requirements for an exemption will be subject to additional testing requirements.
As a contractor, you will receive additional information about how to provide proof of vaccination and what will constitute exemption from this rule from either your contract employer or IDHS administration. Please note that nothing in the Governor’s Executive Order prohibits any entity from implementing vaccination or testing requirements that exceed the requirements of the Executive Order and of the forthcoming additional guidance and information.
