About this file

This is a scholarship application form for the Volusia County Community Assistance 2024 Summer Camp Scholarship Program in Florida. The application collects parent/guardian contact information, household composition data, and details about applying children, including name, date of birth, age, grade level, and foster care status. Applicants must identify their intended summer camp provider, though placement is not guaranteed and registration with the camp is required separately. The program has been operated by the Volusia County Community Assistance division for over 20 years and functions by awarding scholarship vouchers to eligible families that can be redeemed at any approved provider's camp site.

Applicants must submit supporting documentation including proof of household size, recent report cards or progress reports for each child, proof of household income from the last 30 days, and for foster children, a Letter of Assurance. Children age five without report cards may substitute proof of age documentation. Households claiming no income must provide a Declaration of No Income form for each adult member. Foster children do not require proof of household income if all applying children are in foster care. Program funding derives from the Children and Families Advisory Board (CFAB) budget allocation, and the CFAB establishes program requirements and maintains oversight. The County is seeking to partner with an external organization to manage day-to-day program operations while retaining overall program oversight and ensuring compliance with CFAB requirements.
