About this file
This is a Vision Request for Proposal (RFP) workbook and supporting documentation from the County of San Mateo's Benefits Division seeking competitive proposals for comprehensive vision services coverage. The RFP solicits proposals to serve approximately 16,140 covered lives across three plan offerings: VSP Actives Core (4,051 enrollees, self-funded), VSP Actives Buy-Up (1,695 enrollees, self-funded), and VSP Retirees Voluntary (1,100 enrollees, fully insured). The RFP was released to market on February 23, 2026, with proposals due by March 13, 2026 at 5:00 pm. The evaluation timeline includes Committee Meeting #2 on April 7, 2026 to review results and select finalists, Committee Meeting #3 on April 30, 2026 for finalist presentations, and presentation of results to the Benefits Committee on July 23, 2025, with an award date of July 31, 2026. Implementation is scheduled to begin September 1, 2026, with the plan effective date of January 1, 2027. The County requests a minimum three-year ASO rate guarantee through January 1, 2030, with the resulting contract having an initial five-year term and one optional two-year extension exercisable at the County's sole discretion.
Proposers must submit pricing on a net-of-commission basis with no mandatory participation requirements and must provide quotes for both 85% UCR and MAC reimbursement options for out-of-network services based on 24 months of claims data. The incumbent carrier is Vision Service Plan (VSP), with existing agreements expiring December 31, 2026. Minimum qualification requirements include at least ten years of vision coverage experience, demonstrated ability to integrate with Workday and BCC systems, a national vision provider network with 90% or greater geographic access match and at least 60% provider match, experience serving California public agency clients with over 5,000 employees (with three references required), financial rating of A or greater (AM Best, Moody's, or S&P), and professional liability insurance of $1,000,000 with the County named as additional insured. The evaluation process applies Phase 1 criteria weighting financial terms (30 percent), provider network capabilities (30 percent), plan offering flexibility (20 percent), and customer service quality (20 percent), followed by Phase 2 application of local business preference points (10 percent) in accordance with San Mateo County Ordinance Code Chapter 2.89.
