About this file

This is a Bid Opportunity List form template issued by the Wisconsin Department of Transportation in compliance with 49 Code of Federal Regulations Part 26.11. The form is designed to maintain a comprehensive record of all firms participating or attempting to participate on DOT-assisted contracts, including prime contractors, subcontractors, subconsultants, and vendors supplying materials and services. The document requires submission with all bids and proposals and applies to all participants on DOT-assisted projects, regardless of Disadvantaged Business Enterprise (DBE) status. For consulting companies, the list must include all subconsultants who contact the prime consultant and express interest in teaming on a specific DOT-assisted project.

The form requires collection of standardized information for all bidders, including company name, contact information, DBE status, firm age, type of work performed, and annual gross receipts categorized into six brackets ranging from less than $500,000 to $5 million and above. Prime contractors and consultants must complete all columns with available information, while prime contractors are responsible for completing columns for subcontractors and subconsultants based on information they possess or obtain. The form template provides multiple entry rows with identical data collection requirements for each participant, ensuring uniform documentation of firm characteristics and business status across all DOT-assisted contract opportunities in Wisconsin.
