About this file

This is a Bidders List form (Appendix I) issued by the Wisconsin Department of Transportation for the RFP 01-2018 Shared Ride Taxi Services contract opportunity covering Clark County, the Municipality of Stanley, and the City of Neillsville. The form serves as a bid opportunity list required under 49 Code of Federal Regulations Part 26.11, which mandates that the Wisconsin DOT develop and maintain documentation of all firms participating or attempting to participate on DOT-assisted contracts. Proposals were required to be submitted in a sealed package containing one original (clearly identified) and three copies by 4:00 PM on September 29, 2017, to the Clark County Department of Social Services at 517 Court Street, Room 502, Neillsville, Wisconsin 54456. Pricing proposals must be included in a separate sealed envelope with only the original procurement administrator's copy.

All bidders, including prime contractors, consultants, subcontractors, and subconsultants, must complete and submit this bidders list form with their proposals. The form requires disclosure of company name, contact information, DBE (Disadvantaged Business Enterprise) status, firm age, type of work, and annual gross receipts categorized into five ranges from less than $500,000 to $5 million and above. Prime contractors and consultants must complete all columns, while subcontractors and subconsultants require prime contractors to complete columns one and two along with any available information on the remaining four columns. This documentation ensures compliance with federal requirements for tracking participation on DOT-assisted projects regardless of DBE designation.
