About this file

This is a Contractor Security Acknowledgement Form issued by the State of South Dakota Department of Health for compliance with the state's Information Technology Security Policy. The form requires contractors and their employees working on state contracts to acknowledge and agree to protect non-public state technology infrastructure and information assets from unauthorized access, disclosure, modification, deletion, and usage. Contractors must commit to following state and federal regulations regarding data confidentiality and handling, comply with all provisions of the attached IT Security Policy, and promptly report any policy violations to the designated state contact and the BIT Help Desk at 605-773-4357. The acknowledgement establishes that access to state technology infrastructure is a privilege that may be modified or revoked at the discretion of BIT management, and that such access automatically terminates upon contract completion unless otherwise agreed in writing. The signatory certifies authority to commit the entire organization and all employees to the terms outlined in the agreement.

The second paragraph addresses enforcement and operational requirements, establishing that individuals found in violation of the security policy are subject to privilege revocation and potential breach of contract determinations at the state's discretion. The form serves as a binding agreement between the contractor and the State of South Dakota, requiring signature by an authorized representative with the authority to commit the entire company. This acknowledgement form is a standard requirement for the Rural Health Forward contract opportunity, which mandates compliance with the State of South Dakota Information Technology Security Policy Contractor Version 6.0, including federal fingerprint-based background checks, multi-factor authentication for remote access, and strict controls on data handling and production environment usage.
