About this file

This is a Contractor Safety Evaluation Form issued by the City of Lakeland, Florida's Risk Management and Purchasing Department for contractors bidding on work involving systems covered by the Process Safety Management (PSM) Standard. The form requires contractors to provide comprehensive safety documentation including company accident statistics, frequency rates of medical and lost-time injuries compared to national averages for their industry, current Experience Modification Rates (EMR), and details about their safety programs and practices. Contractors must demonstrate they conduct regularly scheduled safety meetings, maintain documented safety and health training programs for new employees, have written safety programs, and perform field safety inspections for regulatory compliance.

All contractors awarded work under this evaluation must provide documentation that employees have received training on fire, explosion, and toxic release hazards specific to their job assignments, including the employee identity, training date, and verification of comprehension. Additionally, contractors are required to prepare a Pre-Work Safety Assessment (PSA) or Job Hazard Analysis (JHA) for all work on PSM-covered systems, which must be reviewed by all personnel involved in the work at its beginning and as needed throughout execution. Any modifications to the PSA or JHA during work must be approved by the senior contractor supervisor on site and either the City of Lakeland Safety Manager, the North McIntosh Plant Superintendent, or their designees. The form requires company officer certification of understanding and compliance with these requirements.
