About this file
This is a Contractor Certification Statements form for the State of Ohio related to a Warrensville Developmental Center generator and transfer switch preventive maintenance solicitation. The document requires bidders to complete certification statements to claim eligibility for domestic source end product preference, Ohio preference (Buy Ohio), or Veteran Friendly Business Enterprise (VBE) preference. Bidders must accurately disclose where products and services are mined, raised, grown, produced, or manufactured, and certify compliance with applicable state and federal procurement regulations. Any bidder submitting false or misleading information in an attempt to receive a bid preference will be immediately disqualified and may face legal action including debarment.
The certification form addresses three primary preference categories. Under the Domestic Preference (Buy American) section, bidders must identify the country of origin for each product or service and certify whether components are produced in the United States. The Ohio Preference (Buy Ohio) section requires bidders to verify whether products are produced in Ohio and whether they maintain significant economic presence within the state through tax payment, Secretary of State registration, and employee residency in Ohio or border states (Kentucky, Michigan, New York, Pennsylvania, and Indiana). The Veterans Preference section requires certification of Veteran Friendly Business Enterprise status as defined by Ohio Administrative Code. The state reserves the right to clarify any submitted information during the evaluation process, and completion of the appropriate certification section is mandatory for bidders to receive their claimed preference.
