About this file
Attachment 3 is a Contractor Certification form related to the Immigration and Nationality Act of 1952, as amended, for the Department of Veterans Affairs (VA) contract for On-Call Vascular Ultrasound Services. The certification requires the contractor to comply with all legal provisions concerning non-immigrant foreign nationals working under the contract, specifically for VA patient referrals.
The document mandates that the contractor shall not knowingly employ illegal aliens or foreign nationals in violation of their immigration status. The contractor must comply with E-Verify requirements and Executive Order 12989. Failure to comply may result in prohibiting non-compliant foreign nationals from working at VA-related service locations and potentially terminating the contract. The contractor is also required to obtain similar certifications from its subcontractors, with a warning that making false statements could lead to prosecution under 18 U.S.C. 1001.
