About this file
This Contract Intake Form is a standardized document required by the Office of Superintendent of Public Instruction (OSPI), State Board of Education (SBE), Professional Educator Standards Board (PESB), and Financial Education Public-Private Partnership (FEPPP) for all potential contractors seeking agreements with these Washington State agencies. All prospective vendors must complete and submit this form prior to contract execution. The form facilitates the collection of essential contractor information including legal business name, doing business as (DBA) name, primary contact details for contract management, and signatory information. OSPI uses the provided contact information to deliver final contracts through DocuSign for electronic signature.
The intake form requires contractors to furnish comprehensive business information including taxpayer identification numbers (SSN for individuals, EIN for entities), Statewide Vendor (SWV) numbers required for state payment processing, current Washington State business licensing status, and disclosure of any prior contract terminations for default. Contractors must also identify whether subcontractors will be utilized and report payments through the state's Access Equity system. The form includes certification opportunities for small, woman-owned, minority-owned, and veteran-owned businesses, with corresponding certification numbers where applicable. Additionally, contractors must disclose any current or former employment relationships with Washington State agencies within the previous 24 months, as this may require consultation with the Executive Ethics Board before contract issuance. All information submitted is certified under penalty of perjury, and contractors are obligated to notify the agency of any subsequent changes.
