About this file
The provided document is a Contract Forms Packet for the City of Livonia, Michigan, containing comprehensive insurance, bonding, and contractual requirements for municipal projects. The packet outlines detailed specifications for insurance certificates (Form 1304A), which must include evidence of public liability, motor vehicle, worker's compensation, and owner's/contractor's protective insurance. Contractors are required to provide a performance bond, maintenance and guaranty bond, and labor and materials bond, each in an amount equal to 100% of the contract price from an "A" rating or better company. The bonds must cover work performance, materials, and guarantee all work for one year after the final estimate.
The insurance requirements specify minimum coverage levels, including $1,000,000 per occurrence for general liability, bodily injury, and property damage, with additional provisions for automotive liability and workers' compensation. Contractors must submit four copies of various insurance certificates and ensure 30-day notice to the City of Livonia in case of policy cancellation. The document includes specific endorsements such as CG 20 10 04 13, which names the City of Livonia as an additional insured, and detailed instructions for policy documentation. These extensive requirements aim to protect the City of Livonia's interests by ensuring comprehensive insurance coverage and financial security for municipal contract work.
