This document is a Contract Amendment (#1) for Contract #CTR070548 issued by the Arizona Department of Child Safety's Office of Procurement, located in Phoenix, Arizona. The amendment involves Averhealth as the primary contractor and expands the company's laboratory testing services across multiple Arizona counties.
The amendment adds new counties to Averhealth's scope of service for Laboratory Testing Services, specifically Substance Use Screening and Genetic/Paternity Testing, effective August 31, 2024. The contract includes detailed pricing for various testing services, such as Tele-Collection (both ADCS location and Client Direct) for oral fluid swab screens with six and nine panel options, as well as DNA testing. Prices range from $52 to $300 per test, with specific rates for different types of screenings including tests for substances like cocaine, marijuana, methamphetamines, opiates, and alcohol. The Tele-Collection payment points are considered temporary and will remain in effect until the Arizona Department of Child Safety determines they are no longer necessary, with the department reserving the right to terminate these payment points with written notice.
