About this file
This document is an Invitation for Bid (Solicitation No. BPM006038) issued by the State of Arizona's Department of Child Safety, Office of Procurement & Contracts, located in Phoenix, Arizona. The solicitation is specifically for Laboratory Testing Services and includes Attachment 9, which is a Conformance Statements form that requires bidders to acknowledge their understanding and compliance with the solicitation's terms.
The Conformance Statements form requires bidders to indicate their compliance with three key areas: the Scope of Work and Pricing, Contract Terms and Conditions, and Solicitation Instructions. Bidders must choose either a "YES" option (full compliance) or a "NO" option (compliance with listed exceptions in specific attachments). If no option is checked, the state will assume the bidder has selected "YES". The document emphasizes that taking exceptions could result in the state rejecting or downgrading the offer during evaluation. The solicitation documents are available online at APP.AZ.gov and include multiple parts, exhibits, and attachments that bidders must carefully review.
