About this file

The document is a Consultant/Contractor Reference Check form from the City of Phoenix Office of the City Engineer's Design and Construction Procurement section for an Environmental/Natural-Cultural Resources/National Environmental Policy Act (NEPA) On-Call Consulting Services contract for calendar years 2026-2028. The procurement seeks qualifications for consulting services, with submissions due via the City's Procurement Portal by 12:00 pm on September 26, 2025. The reference check process is a critical component of the qualification evaluation, where potential contractors must provide three reference checks, with each reference able to score up to 7 points (totaling 21 possible points). References will be asked to evaluate the contractor's performance across several categories including business relationship, team composition, problem-solving, project completion, work quality, innovation, and willingness to rehire.

The reference evaluation form provides a structured methodology for scoring past performance, with ratings ranging from 1 (Unsatisfactory) to 4 (Excellent) across six key performance areas. Completed reference forms must be submitted to soq.referencechecks@phoenix.gov by 12:00 pm Phoenix time on the specified date, with the understanding that failure to submit can negatively impact the proposal. The evaluation becomes a public record and will be used by the Design and Construction Procurement section to help determine contract award. The on-call consulting services are specifically focused on environmental and NEPA-related consulting for the Street Transportation Department, with the contract term spanning calendar years 2026-2028.
