About this file

This is a Consolidated State Law Affidavit required by the City of Gainesville, Florida for the Residential Curbside Food Waste Collection and Processing RFP (PWDA-260027-SM). The City is procuring residential curbside food waste collection and processing services for approximately 29,000 single-family homes, municipal facilities, and designated special events. Contractors must collect containerized food waste from curbside locations weekly and deliver materials to City-approved processing facilities for conversion into reusable compost products, with new carts or containers provided at no cost to residents. The mandatory pre-proposal meeting is scheduled for February 12, 2026, at 10:30 a.m. at City Hall in Gainesville, with questions due by March 3, 2026, at 11:59 p.m., and proposals due by March 19, 2026, at 3:00 p.m. Oral presentations are scheduled for the week of April 27, 2026, with anticipated service commencement on October 1, 2026. The initial contract term runs through September 30, 2027, with options for up to four additional one-year renewal terms. Failure to attend the mandatory pre-proposal meeting results in proposal disqualification, and only one proposal per entity is permitted.

All proposers must execute this affidavit under penalty of perjury and have it notarized before a Florida notary public, certifying compliance with multiple Florida statutes including the Convicted Vendor List (Section 287.133), Scrutinized Companies prohibitions (Section 287.135), E-Verify employment authorization verification (Section 448.095), anti-human trafficking provisions (Section 787.06), foreign countries of concern restrictions (Sections 288.0071 and 287.138), and Israel boycott prohibitions (Section 215.4725). Proposers must demonstrate at least three years of documented experience in residential or commercial food waste collection of comparable size and complexity, provide three references from municipal or governmental clients, and maintain comprehensive safety programs with employee training. Contractors must maintain a living wage of $15.00 per hour with health benefits or $17.25 per hour without benefits, adjusted annually. The evaluation criteria encompasses price (100 points), technical qualifications (100 points), written proposal quality (100 points), vehicle type and fuel efficiency with preference for electric, hybrid, and alternative-fuel vehicles (15 points), and oral presentation performance (100 points), for a total of 415 possible points. The City has not predetermined a budget and may award contracts to multiple vendors based on proposal rankings and available funding.
