The information provided for the prime offeror and each proposed major subcontractor must include the entire company name, company address, CAGE Code, DUNS Number and type of work to be performed by citing the applicable Government PWS subparagraph number. This includes all subcontractors who will be providing critical hardware/services and insert in the percentage which will be performed by subcontractors of the total proposed price. In addition, letters of commitment shall be included for all major subcontractors.

(ii) VOLUME II Factor 2 – Past Performance. This volume shall contain past performance information regarding similar contracts. This volume shall not exceed 25 pages plus five pages for each major subcontractor, excluding Past Performance Questionnaire Forms. Offerors shall submit all Government and/or commercial contracts for the prime offeror and each major subcontractor in performance or awarded during the past three years, from the issue date of this RFP, which are relevant to the efforts required by this solicitation. Relevant efforts are defined as services/efforts that are the same as or similar to the effort (as compared to North American Industry Classification System (NAICS) code 541519) required by the RFP.
Past performance will describe the recency, relevancy and quality of past work efforts as it pertains to current and completed contracts which offerors consider relevant to demonstrate their ability to perform the proposed contract effort. Quality of past work efforts will be demonstrated by assessing quality of services, customer satisfaction, timeliness of performance and business relations.
Offeror is advised that in conducting this assessment the Government reserves the right to use both data provided by the Offeror and data obtained from other sources.
Data concerning the offeror shall be provided first, followed by each proposed major subcontractor, in alphabetical order. The Offeror shall also submit the written consent of its major subcontractors to allow the disclosure of its subcontractor’s past performance information to the Offeror. In addition, letters of commitment shall be included for all major subcontractors for their past performance to be considered.
A subcontractor’s past performance and experience will not be given the same level of consideration as either the prime contractor or the joint venture partner because there is no direct legal relationship between the Government and the subcontractor. The Government will consider the past performance and experience of a subcontractor where the prime contractor provides, in its proposal, evidence of a binding teaming agreement or other contractual agreement which creates legal responsibility on the part of the prime contractor and the subcontractor to enter in to a subcontract. However the level of consideration will depend on the extent to which the proposal demonstrates the prime contractor’s commitment to award a subcontract to the subcontractor and the subcontractor’s commitment to enter in to a subcontract with the prime contractor, including legal accountability for both parties.
Past performance information shall be presented in the following format. The Offeror is encouraged to ensure the information provided is current. Data that is not current will be discarded and not considered. This volume shall be organized into the following sections:

ADDENDUM TO 52.212-2 Evaluation – Commercial Items
A. EVALUATION FACTORS FOR AWARD (Addendum to 52.212-2 Evaluation) as set forth below.
B. BASIS OF AWARD
The Follow-on C4IM requirement in support of the 9th MSC, U.S. Army Reserve, Hawaii, will be solicited as a competitive small business set aside. The Government intends to award a single firm fixed price contract to one small business firm.
The award will be made based on the best overall, best value proposal that is determined to be the most beneficial to the Government, with appropriate consideration given to the three (3) evaluation factors: Technical, Past Performance, and Price. The trade-off process will be used in this procurement to consider award to other than the lowest priced offer. All evaluation factors and sub factors and their relative importance are stated in the solicitation and this source selection plan. The Technical factor is significantly more important than the Past Performance factor, which when combined are approximately equal to the Price factor. To receive consideration for award, a rating of no less than “Acceptable” must be achieved for the Technical factor (to include all subfactors). Offerors will be cautioned that the award may not necessarily be made to the lowest price offered. NOTE: ALL NON-PRICED FACTORS
COMBINED ARE APPROXIMATELY EQUAL TO PRICE.
C. FACTORS AND SUBFACTORS TO BE EVALUATED
The following evaluation factors and subfactors will be used to evaluate each proposal: Award will be made to the offeror whose proposal is most advantageous to the Government based upon an integrated assessment of the evaluation factors and subfactors described below. The Government reserves the right to make award based upon the Price factor in the event that the Technical and Past Performance evaluation results of all the offerors’ proposals are substantially the same.
Factor 1: Technical: The Technical factor is further divided into the following subfactors:
a. Subfactor 1: Key Personnel Qualifications and Experience
b. Subfactor 2: Management and Staffing Plan
These two subfactors are approximately equal in importance.
Evaluation of the offeror’s proposal shall address each Technical subfactor as it applies to the Performance Work Statement (PWS). A detailed explanation of the criteria for the evaluation is set forth in the “Evaluation Approach”, Paragraph D of this section. During evaluation of each proposal, the Government will assign each Technical subfactor an adjectival rating and write a narrative evaluation reflecting the identified findings.
Factor 2: Past Performance: Past Performance Information and Past Performance Evaluation Questionnaire. Each offeror’s past performance will be reviewed as indicated in section D.3, Evaluation Approach, Past Performance Factor, to determine relevancy and confidence assessment.
Factor 3: Price: (NOTE: Price is not scored. Adjectival ratings shall not be used for offerors’ Cost/Price proposals.)
Although not scored or rated, Contract Line Item (CLINs) X001 will require a priced proposal.
Contract Line Item (CLINs) X002 is a FFP estimated amount and no price is required to be provided by the offeror. The FFP amount does not obligate the Government to perform the travel indicated. The Government reserves the right to add travel if necessary.
Factor 1 (Technical) is significantly more important than Factor 2 (Past Performance). When combined, Factors 1 and 2 (Technical and Past Performance combined) are approximately equal to Factor 3 (Price).
The resulting award will be a firm-fixed price contract. Price reasonableness will be utilized in the evaluation of the Firm-Fixed Price effort.
As set forth in FAR 52.215-1 (f)(4), the Government intends to evaluate proposals and award a contract without discussions with offerors (except clarifications as described in FAR 15.306(a), Clarifications and award without discussions). It is the Government’s intent to award a contract based on initial offers received, without discussions or negotiations and is, therefore, seeking initial proposals that contain the offeror’s best terms and pricing. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The offeror’s proposal shall contain sufficient information to enable the Government to evaluate all factors and subfactors. The offeror shall not assume that discussions will be conducted.
To be considered for award, the proposal shall conform to all terms and conditions contained in the solicitation and the prospective contractor determined responsible in accordance with FAR Part 9.1. Non-priced factors and sub-factors are evaluated and scored.
A competitive range will be determined. A competitive range may be established if there will be discussions. Any proposal receiving a rating of “Unacceptable” in any non-priced factor or sub-factor may be ineligible for award consideration and if necessary, will be referred to the Small Business Administration for Certificate of Competency.
At any time prior to selection, including upon receipt of proposals, the Government may exclude a proposal from further consideration for reasons such as (but not limited to): Non-compliance with instructions related to the solicitation is considered non-responsive; the proposal is not technically acceptable (not withstanding determination of competency by the Small Business Administration); or the proposal is not likely to be selected for award.
D. EVALUATION APPROACH
All proposals shall be evaluated by the Source Selection Team (SST).
1. The overarching evaluation approach for all factors and subfactors is as follows:
a. Adequacy of Response. The proposal will be evaluated to determine whether the offeror’s methods and approach have adequately and completely considered, defined, and satisfied the requirements specified in the RFP. The proposal will be evaluated to determine the extent to which each requirement has been addressed in the proposal in accordance with the proposal submission section of the RFP.
b. Feasibility of Approach. The proposal will be evaluated to determine the extent to which the proposed approach is workable and the end results achievable. The proposal will be evaluated to determine the extent to which successful performance is contingent upon proven devices and techniques. The proposal will be evaluated to determine the extent to which the offeror is expected to be able to successfully complete the proposed tasks and technical requirements within the required schedule.
2. The Technical factor is divided into the following subfactors:
Subfactor 1: This subfactor evaluates the offeror’s proposed Key Personnel Qualifications and Experience for the basic contract period and all option years. The Government will evaluate the biographical summaries/resumes submitted to determine the extent to which personnel meet the qualification requirements as identified in the requirements in the table listed under PWS 1.4.13.
“Key Personnel” in accordance with the table at PWS 1.4.13 are those who will work on this requirement and are available on the effective date of the contract, therefore offerors shall NOT include a sample person or potential personnel in their proposal for evaluation. Marginal levels of overall Unacceptable could indicate a lack of understanding concerning mission requirements and may result in the entire proposal receiving an unfavorable rating and/or being eliminated from the competitive range. The evaluation will focus on the offerors’ experience, certifications and resumes provided to determine the ability to have qualified individuals that meet the minimum requirements. The evaluation will use the overarching evaluation approach as mentioned in paragraph D, Evaluation Approach, paragraph 1.
Subfactor 2: This subfactor evaluates the offeror’s proposed Management and Staffing Plan and will be evaluated with major emphasis placed on the overarching evaluation approach identified in paragraph D, Evaluation Approach paragraph 1.
As stated in PWS Paragraph 1.14.6 a phase-in plan shall be included in the proposal submittal.
NOTE: Full Quality Control Plans WILL NOT be evaluated. A full Quality Control Plan will be required if you are awarded a contract and is NOT required with the proposal. If you do provide a plan, indicate it is a draft or a sample from another project. Also, provide the page numbers where the required information/answers to the requirements can be found. If you fail to follow these instructions, your proposal may not be adequately evaluated.
The Technical Factor Ratings, excerpted below in accordance with the Army Source Selection Supplement (AS3), Section 3.1, the Army methodology for evaluating Technical Approach and Related Risk is the Combined Technical/Risk Rating, (see Table 1 below). This methodology considers risk, in conjunction with the strengths, weaknesses, significant weaknesses, uncertainties, and deficiencies in determining technical ratings.
TABLE 1 – COMBINED TECHNICAL/RISK RATINGS
Color Rating Description
Blue Outstanding Proposal indicates an exceptional approach and understanding of the requirements and contains multiple strengths, and risk of unsuccessful performance is low.
Purple Good Proposal indicates a thorough approach and understanding of the requirements and contains at least one strength, and risk of unsuccessful performance is low to moderate.
Green Acceptable Proposal meets requirements and indicates an adequate approach and understanding of the requirements, and risk of unsuccessful performance is no worse than moderate.
Yellow Marginal Proposal has not demonstrated an adequate approach and understanding of the requirements, and/or risk of unsuccessful performance is high.
Red Unacceptable Proposal does not meet requirements of the solicitation, and thus, contains one or more deficiencies, and/or risk of unsuccessful performance is unacceptable.
Proposal is unawardable.
TABLE 6 - Technical Risk Ratings Adjectival Rating Description
Low Proposal may contain weakness(es) which have little potential to cause disruption of schedule, increased cost or degradation of performance.
Normal contractor efforts and normal Government monitoring will likely be able to overcome any difficulties.
Moderate Proposal contains a significant weakness or combination of weaknesses which may potentially cause disruption of schedule, increased cost or degradation of performance. Special contractor emphasis and close Government monitoring will likely be able to overcome difficulties.
High Proposal contains a significant weakness or combination of weaknesses which Is likely to cause significant disruption of schedule, increased cost
3. Past Performance Factor. The Government will evaluate the offeror’s record of past and current performance to ascertain the probability of successfully performing the required efforts of the PWS.
Submission 1: Past Performance Information. Evaluation on the offeror submitted Past Performance Information will be based on the offeror’s ability to demonstrate the depth and breadth to successfully satisfy all task component areas as listed in the PWS.
Offeror to submit Past Performance Information in accordance with instructions in ADDENDUM to 52.212-1 – Instructions to Offerors, PROPOSAL SUBMISSION.
Submission 2: Past Performance Evaluation Questionnaire.
a. Evaluation of past performance shall be in accordance with this plan utilizing the forms and questionnaires set forth in Attachment A.
b. The Government will focus its inquiries on the offeror’s record of performance as it relates to all solicitation requirements, including cost, schedule, performance and management of subcontractors. A significant achievement, problem, or lack of relevant data in any element of the work can become an important consideration in the evaluation process. Therefore, offerors will be reminded to include the most recent and relevant efforts (within the past three years) in their proposal.
