About this file
This document outlines compliance verification documentation requirements for a state contract opportunity in Ohio, specifically detailing the mandatory documentation bidders must submit with their quote. The document requires bidders to provide several key documents, including an Acord Certificate of Liability Insurance, a signed Bidder's Certification Form, and a Contractor/Subcontractor Affirmation and Disclosure Form. There are critical prohibitions detailed, including restrictions on purchasing services provided outside the United States and a specific ban on making purchases from or investments in Russian institutions or companies.
The document mandates additional compliance documentation such as an Affirmative Action Program Verification Form (AAPV), Secretary of State Charter/Registration Number, current Bureau of Workers' Compensation (BWC) Certification, and applicable minority or specialty trade certifications (MBE, EDGE, VBE, WBE). The state reserves the right to recover funds paid for services performed outside the United States without proper waiver, and contractors must explicitly affirm their understanding of these requirements. All compliance verification documents must be submitted as a separate document from the bidder's certificate and quote, with potential contractors required to complete and potentially revise an Affirmation and Disclosure Form if service locations change during the contract period.
