This is a combined synopsis/solicitation for commodities prepared in accordance with the format in FAR

Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will NOT be issued.

This is a competitive Request for Quote (RFQ) and the solicitation number for this requirement is

FM2823-25-Q-6001. The Operational Contracting Division, Air Force Test Center at Eglin, AFB, Florida, requires an Infant Security System with installation in accordance with the Statement of Objectives

(SOO) and Requirements List dated March 28, 2025. This acquisition is under Federal Acquisition

Regulation (FAR) Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition

Procedures, as supplemented. This procurement will be conducted as a 100% Small Business set-aside;

please identify your business size in your response based upon this standard. Award will be made to the lowest priced, responsible Offeror whose offer conforms to all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements. There will be no advanced payments.

The North American Industry Classification System code (NAICS) is 561621 – Security Systems

Services (except Locksmiths) and Product Service Code (PSC) 6350 – Miscellaneous Alarm, Signal and

Security Detection, with a small business size standard of $25,000,000. A firm-fixed priced purchase order will be awarded.

The Offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.

The Government is not responsible for any costs incurred or associated with preparation and submission of an offer in response to this solicitation. NO OFFERORS WILL BE COMPENSATED FOR

SUBMITTING AN OFFER.

REQUIREMENT: Infant Security System and Installation System. The Infant security systems installed must be turnkey and hold an active Authority to Operate (ATO) as granted by CSTAR.

FOB Destination Shipping Requested.

The Contracting Officer has determined that there is a high probability of adequate price competition for this acquisition. Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists no additional price information will be requested. However, if at any time during the evaluation the Contracting Officer determines that adequate price competition no longer exists, Offerors may be required to submit information to the extent necessary to determine the price fair and reasonable.

BASIS FOR AWARD: In accordance with FAR 13.106-1(a)(2)(i) the offerors will be evaluated on a

BEST VALUE basis and the award will be made to the responsible quoter whose quote, conforming to the solicitation represents the best value to the government. In no particular order of significance, the

Page | 2 following factors shall be used to evaluate quotes:

1. Technical capability of the item offered to meet the governments’ requirement

2. Price

3. Past Performance/Evidence of recent experience in similar type and scope

4. Supplier Performance Risk System (SPRS) Rating (IAW DFARS 252.204-7024)

EVALUATION PROCESS:

Award shall be made to the offeror providing the best value to the Government, as determined under FAR

Part 13.106-2(b) (Evaluation of Quotations or Offers). The government is not obligated to determine a competitive range, conduct discussions, solicit final revised quotes, or use other techniques associated with FAR Part 15. Please read each section below carefully for the submittals and information required as part of the evaluation. Failure to provide all requested information shall be considered non-compliant and your quote could be removed from the evaluation process. Award will be made to the responsible offeror whose conforming offer is most advantageous to the Government, considering price and other factors.

The following factors shall be used to evaluate offers:

1. Price (Follow these instructions):

Offerors must submit firm-fixed price quotations that meet the requirements in this RFQ, including all information and Contract Line-Item Numbers (CLINs) specified in Attachment 4 (Quote

Submission). Quotes without pricing for the specified CLINs will be deemed unacceptable and ineligible for award.

2. Past Performance Relevancy:

The Offeror must provide one (1) to three (3) references of similar work experience using Attachment

5, Past Performance Relevancy Worksheet. These references demonstrate the Offeror's capacity to meet the requirements outlined in the Statement of Objectives. The Contracting Officer may contact these references for verification. The Government may also gather performance information from other sources for evaluation purposes.

3. Technical Evaluation:

Offerors will be evaluated on their experience and capability to provide the requested services efficiently and in accordance with the Statement of Objectives (SOO). Offerors must demonstrate their corporate experience, approach to meeting all customer’s requirements and objectives outlined in the SOO and requirements list, and that their technicians meet the qualification standards.

If any portion of this work will be subcontracted, please provide the name and address of each subcontractor and a description of their planned contribution.

SITE VISIT (Strongly Encouraged)

A site visit is strongly recommended for offerors to assess the installation location and gather information for accurate quote preparation. Attending the site visit will provide a comprehensive understanding of installation conditions and potential performance constraints.

While not mandatory for quote submission, attendance is highly encouraged. All questions arising from the site visit must be submitted in writing to the Contracting Officer by Thursday, April 25th, 5:00 PM CST.

This deadline applies to all offerors, regardless of site visit attendance. Responses will be distributed to all interested parties.

SUBMISSION REQUIREMENTS

In accordance with, FAR 52.212-1(b)(4), offerors are reminded that as a minimum, offers must show a technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary.

Offerors are expected to include all labor, materials, equipment, and training necessary for a fully operational Infant Security System, including hardware, software, training and installation.

The government will not accept refurbished/used equipment.

Offers must list CAGE code, Company name, POC, and phone number. If you need to obtain or renew a

CAGE code, please visit https://sam.gov/. Lack of registration in SAM database will make an offeror ineligible for award. Vendor must also be registered in Wide Area Work Flow to receive payment through electronic funds transfer. If you are not currently registered, please visit the following website https://wawf.eb.mil/ to complete your registration.

Offerors must submit quotations for all items listed. Partial offers will not be considered for award. Offerors company generated quotes will be accepted but must include all requested information with in the RFQ.

REQUIREMENT

The 96th Medical Group requires a TURNKEY purchase and installation of an Infant

Security System, including the deinstallation, removal and disposal of the existing infant abduction prevention system.

ESSENTIAL CHARACTERISTICS:

This system is essential for ensuring the safety and security of newborns within our facility. It provides continuous real-time monitoring, ensuring that each infant remains within designated secure areas. The system automatically locks doors and triggers alarms in the event of an unauthorized removal attempt, significantly reducing the risk of infant abduction. Additionally, it integrates with hospital security protocols, offering seamless communication with staff and emergency responders. Features such as tamper detection, staff override capabilities, and integration with fire alarm systems ensure both security and operational efficiency while maintaining compliance with regulatory standards.

Hospital Infant Security System must include the following essential features:

• Unique Identification: Each infant must be fitted with a tamper-proof identification tag linked to a real-time tracking system.

o RFID or Wi-Fi-enabled infant tags with unique IDs o Tamper-resistant, adjustable bands for infants

• Location Tracking: The system must continuously track the infant's location within the facility with high accuracy o Real-time location tracking within the hospital premises

• Parent/Guardian Matching: The system should be able to match an infant with their parent(s)/guardian(s) and verify their identity before releasing the infant.

The system should alert staff if an incorrect infant/guardian match is made.

o Dual-band system linking infant and mother tags o Audible and visual alerts for mismatches o Secure verification process for mother-infant pairing

• Real-Time Alerts: The system should generate real-time alerts for unauthorized movement, tampering with tags, or attempts to disable the system.

• Protection Area Lockdown: The system should be able to initiate a lockdown within the protection area in the event of a suspected abduction.

• Access Control Integration o Automatic door locking in case of an infant abduction attempt o Restricted access to sensitive areas (NICU, maternity wards) o System must have Authorization to Operate (ATO) as determined by government Consolidated System Tracking and Reporting (CSTAR)

• Tagging & Registration o Each newborn is assigned a unique, tamper-resistant tag at birth.

o Tags are registered in the system o Automatic alerts if a tag is removed or tampered with.

• 24/7 Continuous Real-Time Monitoring o The system must be operational at all times with minimal downtime o Continuous tracking of tagged infants within designated secure zones.

o Immediate system alerts if an infant is detected near an unauthorized exit.

• Hospital Environmental Conditions o Operates effectively in temperature ranges between 50°F – 90°F o Humidity tolerance of 20% – 80% (non-condensing)

• Exit Point Security o Readers installed at all designated exit points to detect unauthorized movement.

o Automated lockdown of exits upon unauthorized tag detection.

o Audible and visual alarms triggered in case of security breaches.

• Alarm & Emergency Response Integration o Instant notification to hospital security and staff in the event of a breach.

o Integration with fire alarm systems to allow a controlled exit delay of 15 seconds per NFPA 101 Paragraph 7.2.1.6.1 and 7.2.1.6.2 before unlocking doors for emergency evacuation.

• Compliance & Safety Standards o System complies with hospital security policies and regulatory requirements.

o Meets HIPAA requirements for patient data security o Complies with FDA and Joint Commission hospital safety regulations o Uses non-toxic, hypoallergenic materials for infant wearables o Comply with Federal, State, and installation codes, standards, and guidelines to include those outlined in the National Fire Protection

Association (NFPA), Unified Facilities Criteria (UFC), American Hospital

Association (AHA), Occupational Safety and Health Association (OSHA), American Society of Mechanical Engineers (ASME), National Electric Code

(NEC), American Society of Heating, Uniform Federal Accessibility

Standards (UFAS), American Disabilities Act (ADA), American National

Standard Institute (ANSI), and MIL Handbook 1191.

• Emergency Override Protocols o Manual override options for authorized personnel o Ability to disable or reset alarms in case of false alerts o Secure audit trail logging for all system interactions

• Power & System Reliability o Uninterrupted operation with backup power in case of an outage.

• Alarm Systems: Integration with automated door locking system and fire alarm system to allow controlled egress in the event of a fire alarm.

o Instant alerts for unauthorized tag removal or tampering o Exit alarms for unauthorized infant movement near exits o Integration with hospital-wide paging and alert systems

• Transfer Capabilities: Must have the ability to control transfer of infant from one protection area to another without triggering alarm. Transfer must automatically locate and add infant to the new protection area upon entering the new space.

• System Integration o Compatibility with hospital security, access control, and nurse call systems o Centralized monitoring software with real-time updates o User-friendly interface for medical and security personnel

• Scalability o Support for multiple infants and caregivers in the system

• System Performance Testing: Thorough testing is required to guarantee the system's accuracy, reliability, and response time.

• User Acceptance Testing: Testing with end-users (staff, parents/guardians) to ensure usability and gather feedback.

• User Training: Minimum 8 hours onsite user training. 8 hours of virtual training.

Access to online user training videos and other media.

• Regular System Audits: Annual audits should be conducted to evaluate the system's effectiveness and identify areas for improvement. Report will be made available along with recommendations to rectify any discrepancies in the systems performance.

• Technology: Must use RFID/Wi-Fi technology that is located on a customer provided server. System requires an active Authority to Operate (ATO) on the

MedCOI system as determined by CSTAR.

• Updates and Maintenance: Vendor must be able to access system remotely for required updates and system maintenance.

• Tag Battery Life & Maintenance o Long-lasting battery (minimum 1-year lifespan) o Low-battery notification alerts o Easy replacement process for infant and staff tags

OPERATION CONDITIONS:

To ensure the effectiveness and reliability of the infant security system within a hospital setting, the following operating conditions must be met:

The system must function under the following conditions

• Network Infrastructure o MedCOI integrated; Operates on a secure RFID or Wi-Fi network with encrypted communication.

o Regular system maintenance and testing to ensure optimal performance.

o Updates and Maintenance: Vendor must be able to access system remotely for required updates and system maintenance.

o By meeting these operating conditions, the infant security system ensures a safe and controlled environment, minimizing the risk of infant abduction while allowing for emergency preparedness.

• Service Requests: Vendor must provide support for the end user. In the event of an emergency support request, a response must be given within one hour;

vendor must have a plan to resolve in two hours; vendor must resolve the event within eight hours. In the event of a non-emergency support requests, a response must be given.

• Training & Usability o Intuitive software interface for medical staff o Training materials and on-site training for personnel

1.0 BACKGROUND & PURPOSE

The 96th Medical Group (MDG) at Eglin AFB, Florida, requires a TURNKEY Infant Security System to replace the existing system in the Labor & Delivery (L&D) and Multi-Service Unit (MSU). The current system experiences frequent failures, causing security risks and operational inefficiencies.

The new system must provide real-time monitoring, automated security responses, and seamless integration with hospital security protocols to ensure the safety of newborns. Compliance with Joint

Commission (TJC) standards (EC.02.01.01 EP 9) is required.

2.0 SCOPE OF WORK

The contractor shall provide all labor, materials, equipment, and expertise to install a fully operational

Infant Security System. The system must:

• Integrate with fire alarm and automated door systems to prevent unauthorized removal.

• The System requires an active Authority to Operate (ATO) on the MedCOI system as determined by CSTAR

• Use RFID or Wi-Fi tags to secure infants and prevent unauthorized exits.

• Begin work with a post award site visit within three (3) weeks of contract award and complete installation within three (3) months.

The contractor shall:

• Provide a new Infant Security Monitoring System for L&D (3rd floor) and MSU (4th floor).

• Deinstall and dispose of the existing system while maintaining security during transition.

• Conduct a site visit before starting any work to obtain an Infection Control Risk Assessment

(ICRA) from the Infection Control Officer and a Pre-Construction Risk Assessment (PCRA) from

Facilities Management

• Ensure the existing system remains operational until the new system is fully functional.

• Manage transportation, labor, and necessary supplies for installation.

• Ensure compliance with manufacturer specifications, hospital safety requirements, and facility policies.

• Conduct a site survey and facility layout assessment.

• Develop an implementation plan and schedule.

3.0 PERFORMANCE OBJECTIVES

The Infant Security System must meet the requirements outlined in the Military Treatment Facility (MTF)

Requirements Document to ensure compliance with security, operational, and technical standards. The system shall include:

• Tamper-resistant RFID/Wi-Fi infant security tags with unique identification and real-time tracking.

• System requires an active Authority to Operate (ATO) on the MedCOI system as determined by

CSTAR

• Secure exit control points with automated alarm triggers integrated with fire and door systems.

• Minimal disruption to hospital operations during installation.

• Risk mitigation plan for installation challenges.

• Automated alerts for security breaches, unauthorized exits, or system malfunctions.

• Training: Minimum 8 hours onsite, 8 hours virtual, plus access to online training materials.

6.0 DELIVERABLES & KEY MILESTONES

The contractor shall provide:

• Fully operational Infant Security System (hardware & software).

• Installation, testing, and commissioning.

• Comprehensive staff training and user guides.

FAR 52.212-1 INSTRUCTIONS TO OFFERORS - COMMERCIAL PRODUCTS AND

COMMERCIAL SERVICES (NOV 2021)

(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code(s) and small business size standard(s) for this acquisition appear elsewhere in the solicitation. However, the small business size standard for a concern that submits an offer, other than on a construction or service acquisition, but proposes to furnish an end item that it did not itself manufacture, process, or produce is 500 employees if the acquisition—

(1) Is set aside for small business and has a value above the simplified acquisition threshold;

(2) Uses the HUBZone price evaluation preference regardless of dollar value, unless the offeror waives the price evaluation preference; or

(3) Is an 8(a), HUBZone, service-disabled veteran-owned, economically disadvantaged women-owned, or women-owned small business set-aside or sole-source award regardless of dollar value.

(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. It is at the discretion of the Contracting

Officer if a late offer will be accepted. As a minimum, offers must show—

(1) The solicitation number;

(2) The time specified in the solicitation for receipt of offers. Not required;

(3) The name, address, and telephone number of the offeror;

(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;

(5) Terms of any express warranty;

(6) Price and any discount terms;

(7) "Remit to" address, if different than mailing address;

(8) A completed copy of the representations and certifications at Federal Acquisition Regulation

(FAR) 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);

(9) Acknowledgment of Solicitation Amendments. Not required;

(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and

(11) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.

(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

(d) Product samples. Not required.

(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with FAR subpart 4.10), or alternative commercial products or commercial services for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.

(f) Late submissions, modifications, revisions, and withdrawals of offers.

(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.

(2) (i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition, and its in the

Government's best interest to accept the late offer.

(C) If is the only quote received.

(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.

(3) Time and Date Offer Due being marked on quote: This section does not apply.

(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.

(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.

(g) Contract award. The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The

Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.

(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.
