a) Technical Capability. The testing services provider must provide and use a PCR test that is Food and
Drug Administration (FDA) approved (or on the FDA emergency use approval list found here). The test must be performed by properly trained medical personnel who will conduct on-site retrieval of the nasopharyngeal, or oropharyngeal biological, samples for testing. All contractor personnel conducting the tests must be provided with Personal Protective Equipment (PPE) for retrieving the biological samples in accordance with the Centers for Disease Control and Prevention (CDC) and Occupational Safety and Health Administration (OSHA) guidelines for “Preferred PPE.” NOAA requires that all personnel conducting the tests use N95 high-grade masks, face shields, nitrile gloves, and protective gowns. Additional information regarding the required PPE can be found here. The tests may either be processed using a PCR analyzer on-site or sent to a local laboratory for processing results within 48 hours of receiving samples.
Firms must identify the availability of tests described above (PCR), FDA Approval, percentage accuracy (95%+ sensitivity and 98%+ specificity required at a minimum), as well as any relevant details regarding shipment/receipt of items.
It is preferred by the Government for all testing and support services to be performed on-site and responses must specify the intended location, process of conducting testing, as well as time for processing and providing results (less than 48 hours required at a minimum). If vendors recommend an alternate location, the following details must be provided: the process of transporting sample tests to laboratory (must confirm the laboratory is certified under Clinical Laboratory Improvement Amendments of 1988 (CLIA), 42 U.S.C. § 263a to perform high complexity tests as an “authorized laboratory”), list of nearby location(s) within twenty (20) miles of each specified location herein and details of facility. In addition, proof of insurance, should loss or damage occur, must be provided with response to this RFQ.
b) Past Performance. Past performance will be used as a means of evaluating the relative capability of the
Contractor to successfully meet the requirements of this solicitation.
The quoter is required to identify past or current contracts (including Federal, State, and local Government and/or commercial) for efforts similar to the requirement herein. The Government may consider this information, as well as information obtained from any other sources, when evaluating the quoter’s past performance. Quoters lacking relevant past performance history, or for whom past performance information is either not available or has not been submitted to the Government, will receive a neutral rating for past performance. A neutral rating will not be evaluated favorably or unfavorably. However, the submission with no relevant past performance history, while rated neutral in past performance, may not represent the most advantageous submission to the Government and thus, may be an unsuccessful submission when compared to the submissions of other Vendors having an satisfactory (or better) past performance.
The quoter shall limit this information for no more than three (3) contracts performed within the last three (3) years, each listed separately. The quoter is to provide this information in the following format for each contract within Volume I, Section b) Past Performance:
a. Contract Number and two (2) points of contact (including a main point of contact, and alternate, with telephone numbers and email addresses)
b. Dollar Value of the entire contract
c. Type of service performed and whether Prime or Sub Contractor
d. Contract performance period of performance (actual performance dates upon completion to include start date and completion date)
e. Contract Type
The Past Performance Information Form, Attachment II is provided for Quoters to submit the information requested under points a-e (above) for each Past Performance record submitted. This information for each past performance reference shall be included in the quoter’s submitted Volume I, Past Performance portion. In the description of work, a detailed explanation demonstrating the similarity https://www.fda.gov/medical-devices/emergency-situations-medical-devices/emergency-use-authorizations https://www.cdc.gov/coronavirus/2019-ncov/downloads/A_FS_HCP_COVID19_PPE_11x17.pdf of the contract (in terms of scope and magnitude) must be provided, to determine relevancy to the requirement herein.
The Past Performance Questionnaire, Attachment III is provided for quoter’s to utilize and send to clients/customers for whom work has been performed for Past Performance records that are not listed in CPARS. The evaluation criteria and ratings definitions mirror the ratings utilized in CPARS.
Ensure correct phone numbers and e-mail addresses are provided for the client point of contact.
Completed Past Performance forms and questionnaires shall be submitted by the client directly to the Contract Specialist, Jennifer Peters (Jennifer.Peters@noaa.gov) via email no later than 2:00 PM ET, June 26, 2020.
Note: It is the responsibility of the Quoter to ensure references intend to respond by utilizing the Past Performance Questionnaire and ensure the completed questionnaires are submitted by the closing date of the solicitation.
Contractors shall assume that the Government has no prior knowledge of them and that it will base its evaluation on the information presented in the Past Performance Proposal; as such, Past Performance must be fully self-sufficient and provide all necessary information.
Failure to submit the Past Performance Information Form by the date and time cited above may be cause for rejection and the proposal receiving no further consideration.
Volume II - Price Vendors shall submit Firm-Fixed Price (FFP) quotes, including cost for delivery FOB Destination, in response to this RFQ in U.S. dollars, rounded to no more than two (2) decimal places. The total cost should include the total solution to include: testing kits, shipment (if needed), administration of the test (if needed) or location within 20 miles of locations for testing, and providing results.
Pricing shall be submitted for all line items included in this solicitation. The vendor provided pricing for each CLIN will be added together to derive the overall evaluated price. Price will be evaluated for reasonableness.
The Government anticipates that the contract to be awarded will be based upon adequate price competition. However, in order to determine that offered prices are fair and reasonable, the Government reserves the right to request that the quoter provide further information to support proposed prices.
Volume III – Completed Provisions Quoters must provide all completed provisions herein within this volume of the submitted response.
(a) NAICS code and small business size standard. The NAICS code(s) and small business size standard(s) for this acquisition appear elsewhere in the solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show—

Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender’s request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with subpart 4.10 of the Federal Acquisition Regulation), or alternative commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.
(f) Late submissions, modifications, revisions, and withdrawals of offers. (1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2) (i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition;
and-

(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government’s control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.
(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest;
accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.

(End of provision)
B) FAR 52.212-2 Evaluation—Commercial Items (OCT 2014) applies to this acquisition with the following addenda:
The quote shall be evaluated in a best value tradeoff format. The following factors shall be used to evaluate quotes: (i) technical capability of the items quoted to meet the Government requirement; (ii) past performance; and (iii) price.
Technical capability is the most important factor, and is more important than past performance. However, the combination of both non-price factors (Technical Capability and Past Performance) when combined, are significantly more important than price. The Government will not award at a significantly higher price only to achieve slightly high technical capability or performance. The degree of importance of the quoted price will increase as the non-price factors among responses become equal. Vendors with no record of past performance will be assigned a neutral rating for this factor.
(a) Factor 1: Technical Capability: Technical information submitted for this evaluation factor will be evaluated to verify that the equipment and associated items quoted are in accordance with the SOW.
(b) Factor 2: Past Performance: The Government will evaluate past performance using the references provided within the vendor’s quote; however, the Government reserves the right to obtain additional information from other sources outside of those included with the quote. The evaluation will consider the vendor’s past performance on relevant contracts with government and/or commercial customers; points of contact provided within the quote may be contacted. Where there is no record of past performance, the vendor will be evaluated neither favorably nor unfavorably. Superior performance ratings on relevant contracts may be considered more favorably in the evaluation.
(c) Factor 3: Price: The quoted price is to include all items, supervision, labor, services, training, materials, packing, shipping, delivery, travel, taxes, fees, spare parts, standard commercial warranty and any associated services and items to provide the services in accordance with the SOW. The price shall be evaluated to determine fairness and reasonableness. The total evaluated price of the quoter will be determined by totaling the proposed prices of all CLINS herein.
(End of Provision)
C) FAR 52.212-3 Offeror Representations and Certifications – Commercial Items (MAR 2020) applies to this acquisition with the following addenda:
Quoters shall include a completed copy of 52.212-3 with quotes. All clauses shall be incorporated by reference in the order.
The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov.
