The Government reserves the right to conduct discussions if determined necessary by the Contracting Officer.
I. TECHNICAL PROPOSAL REQUIREMENTS
Factor 1 - Past Performance
Offerors are not required to submit separate past-performance references unless specifically requested by the Contracting Officer. The Government will evaluate available recent and relevant past-performance information from official Government sources, including CPARS, and may consider other reasonably available past-performance information.
Recency: Past performance generally will be considered recent when the applicable performance occurred or was completed within the twenty-four (24) months preceding the solicitation closing date.
Relevance: The Government will consider the degree to which prior performance is similar to the present requirement in areas such as healthcare staffing, nursing or clinical services, staffing volume, complexity, scope, and performance environment.
An offeror without a record of relevant past performance, or for whom sufficiently relevant past-performance information is unavailable, will not be evaluated favorably or unfavorably solely on that basis and will receive a neutral/unknown past-performance assessment.
Factor 2 - Technical Approach
The offeror shall provide a technical approach demonstrating its ability to successfully perform the PWS. At a minimum, the approach shall address:
1. Providing and maintaining required Emergency Department RN coverage;
2. Recruiting and providing qualified, licensed, certified, credentialed, and privileged RNs;
3. Maintaining continuity of services;
4. Providing qualified replacement personnel;
5. Meeting patient-care and clinical-documentation requirements;
6. Meeting credentialing, licensing, certification, background investigation, and orientation requirements; and
Combined Synopsis | Page 5
7. Successfully completing transition/start-up requirements.
A statement merely promising to comply with the PWS, without explaining the offeror's approach, will not be sufficient.
Factor 3 - Management Approach
The offeror shall provide a management approach demonstrating its ability to manage the contract successfully. At a minimum, the approach shall address:
1. Contract management and oversight;
2. Recruitment and retention;
3. Staffing and scheduling;
4. Supervision and management of Contractor personnel;
5. Contingency staffing and continuity of operations;
6. Replacement of unavailable personnel;
7. Coordination with Government personnel while maintaining the non-personal nature of the services; and
8. Management of credentialing and other personnel qualification requirements.
A statement merely promising to comply with the PWS, without explaining the offeror's management approach, will not be sufficient.
Factor 4 - Price
The offeror shall submit a completed Price Schedule/Bid Sheet. The Government will evaluate the Total Evaluated Price as described in Section G. The Government may use one or more price-analysis techniques to determine whether the offered price is fair and reasonable, including comparison of proposed prices received, comparison with the Independent Government Estimate, comparison with historical prices for the same or similar services, and other price-analysis techniques permitted by the FAR. The Government may consider whether pricing demonstrates an adequate understanding of the requirement.
J. BASIS FOR AWARD
The Government intends to make one Firm-Fixed-Price award to the responsible, eligible ISBEE offeror whose proposal represents the Best Value to the Government using a tradeoff process. The Government will evaluate the relative merits of the non-price factors and price in accordance with the evaluation criteria stated in this solicitation.
The evaluation factors are: (1) Past Performance, (2) Technical Approach, (3) Management Approach, and (4) Price.
Past Performance, Technical Approach, and Management Approach, when combined, are more important than Price.
The Government will evaluate strengths, weaknesses, deficiencies, and performance risk associated with the non-price factors. A proposal must satisfy all material solicitation requirements to be eligible for award.
Price will be evaluated for reasonableness, completeness, and accuracy using the Total Evaluated Price described in Section G.
Combined Synopsis | Page 6
The Government may select a higher-priced proposal when the evaluated benefits of the non-price factors are determined to warrant the associated price premium. Conversely, the Government may select a lower-priced proposal when the additional benefits of a higher-priced proposal do not justify the price premium.
The tradeoff decision will be based on an integrated assessment of the proposals and will document the rationale for the selection decision.
Adjectival Ratings. Technical Approach and Management Approach will be evaluated using the following adjectival rating definitions:
ADJECTIVAL
RATINGS DESCRIPTION
Outstanding Greatly exceeds all minimum requirements of the criteria; has a high probability of success; contains no weaknesses or deficiencies.
Good Exceeds all the minimum requirements of the criteria; has an above average probability of success;
contains no significant weaknesses and only minor correctable weaknesses exist.
Acceptable Meets all the minimum requirements of the criteria; has an average probability of success; and no significant weaknesses and no deficiencies.
Marginal Fails to meet one or more of the minimum requirements of the criteria; low probability of success;
major weaknesses and/or significant number of deficiencies exists.
Unacceptable Fails to meet any of the minimum requirements of the criteria; proposal needs major revisions; very low probability of success.
Factor 1 - Past Performance
The Government will evaluate available recent and relevant past-performance information to assess the Offeror's likelihood of successful performance. The evaluation may consider quality, timeliness, customer satisfaction, management effectiveness, staffing reliability, and performance on similar healthcare staffing or clinical-service requirements.
An Offeror without a record of relevant past performance, or for whom sufficiently relevant past-performance information is unavailable, will receive a neutral/unknown assessment and will not be evaluated favorably or unfavorably on that basis.
Past Performance will be assessed separately for recency, relevance, quality, and performance risk and will not receive the adjectival ratings used for Technical Approach and Management Approach.
Factor 2 - Technical Approach
The Government will evaluate the degree to which the Offeror's technical approach demonstrates a clear, feasible, and effective method for satisfying the PWS, including maintaining required Emergency Department RN coverage, recruiting and providing qualified personnel, ensuring continuity of services, providing replacement personnel, and successfully completing transition/start-up requirements.
The Government will assign an adjectival rating of Outstanding, Good, Acceptable, Marginal, or Unacceptable to the Technical Approach factor using the definitions above. The evaluation will consider the relative strengths, weaknesses, deficiencies, and performance risks associated with the proposed technical approach.
Factor 3 - Management Approach
The Government will evaluate the effectiveness of the Offeror's management approach for contract oversight, recruitment and retention, staffing and scheduling, supervision of Contractor personnel, contingency staffing, continuity of operations, replacement staffing, credentialing, and coordination with Government personnel while preserving the non-personal nature of the services.
Combined Synopsis | Page 7
The Government will assign an adjectival rating of Outstanding, Good, Acceptable, Marginal, or Unacceptable to the Management Approach factor using the definitions above. The evaluation will consider the relative strengths, weaknesses, deficiencies, and performance risks associated with the proposed management approach.
Award Selection
Award will be made to the responsible, eligible ISBEE offeror whose conforming proposal represents the Best Value to the Government, price and other factors considered. The Government may award to an Offeror other than the lowest-priced Offeror when the evaluated benefits of the non-price factors justify the associated price premium. The Government intends to award without discussions and reserves the right to conduct discussions if determined necessary by the Contracting Officer.
K. PERIOD FOR ACCEPTANCE OF OFFERS
The offeror agrees to hold the prices and terms contained in its offer firm for thirty (30) calendar days from the solicitation closing date, unless a different period is expressly accepted by the Government.

The Standing Rock Service Unit (SRSU), located within the Great Plains Area Indian Health Service, provides healthcare services to eligible American Indian and Alaska Native beneficiaries.
PWS 2
Emergency Department Nursing Services
The Fort Yates Hospital (FYH) is a rural critical access hospital and Level IV Trauma Center located in Fort Yates, North Dakota. The facility provides emergency medical services requiring continuous nursing support to ensure safe, effective, and timely patient care.

1.2 SCOPE
The Contractor shall provide professional Emergency Department nursing services to support the healthcare mission of the Standing Rock Service Unit.
The Contractor shall provide qualified registered nursing personnel necessary to support emergency patient care, including patient assessment, treatment support, monitoring, documentation, care coordination, and other nursing services within the scope of practice.
The Contractor shall provide sufficient qualified personnel to maintain continuous Emergency Department coverage throughout the period of performance.
The Contractor shall provide complete nursing services as described herein. The Government is purchasing the required nursing service capability and availability and is not purchasing individual labor hours.
The Contractor shall maintain responsibility for staffing, scheduling, supervision, and management of Contractor personnel while ensuring compliance with all requirements of this PWS.
1.2.1 Duties and Responsibilities
The Contractor shall provide Emergency Department nursing services in accordance with Section 5.0, Performance Requirements.

Standing Rock Service Unit Fort Yates Hospital 10 Standing Rock Avenue Fort Yates, ND 58538
The Contractor may be required to perform services within other designated areas of the facility when necessary to support patient care operations.
1.2.3 Period of Performance
The period of performance shall consist of a four (4)-month Base Period plus fourteen (14) four-month Option Periods, for a total potential period of sixty (60) months (five years).
• The anticipated start of performance is approximately forty-five (45) calendar days after contract award.
PWS 3
Emergency Department Nursing Services
• The actual dates for the Base Period and each Option Period will be established in the resulting contract.
• The Government reserves the right to exercise Option Period RN CLINs in accordance with the terms and conditions of the resulting contract.
1.2.4 Base Period and Option Period Structure
The requirement consists of up to eight (8) separately exercisable RN positions. Each RN position has a four-month Base Period CLIN and fourteen separately exercisable four-month Option Period CLINs. All RN position CLINs within the same performance period shall be priced using the common RN/MOS unit price established for that performance period.
The Base Period provides for up to thirty-two (32) MOS, representing a maximum of eight RN positions for four months each. The actual number of RN position CLINs awarded or funded for the Base Period will be identified in the resulting contract.

• A potential aggregate of approximately 24,960 nursing hours annually when all eight RN positions are required at that level.
The staffing-hour requirements above establish required service capacity and performance expectations.
They do not change the contract unit of issue or pricing basis: the Government is purchasing RN Months of Service under the applicable CLINs and is not purchasing individual labor hours.
The Government may experience fluctuations in staffing requirements based on operational needs, patient volume, permanent staffing levels, and mission requirements. The Contractor shall maintain sufficient qualified personnel to ensure uninterrupted Emergency Department operations.
