About this file
This is a Functional Requirements Document (FRD) for the Cognitive Monitoring Tool, issued by the Defense Health Agency under the Department of Defense. Version 0.01 dated April 2026, the document establishes requirements for a modern cognitive monitoring platform supporting the entire Joint Force. The FRD is marked CUI//PROCURE and contains two distinct capability phases: a Minimum Viable Capability (MVC) for urgent fielding and full operational capability (FOC) requirements for an enduring enterprise solution.
The MVC requires three mandatory, non-negotiable capabilities: zero client footprint delivery via a secure web URL accessible from any modern browser without software installation; unsupervised self-administration designed for Service members to complete testing without a trained proctor; and proven enterprise scalability with documented evidence supporting minimum 35,000 simultaneous sessions on a currently fielded, battle-tested platform. The FOC requirements define a comprehensive system establishing unique longitudinal cognitive health records for individual Service members with baseline assessments and post-injury comparative testing across four core cognitive domains: Attention/Concentration, Memory, Processing Speed, and Executive Function. Additional FOC capabilities include a customizable task library for tailored assessment batteries, modern standards-based APIs for secure data exchange with Department of War enterprise systems including MHS GENESIS and ILER, ingestion of historical data from the legacy ANAM system, role-based dashboards and reporting tools for clinicians and commanders, enforcement of a Common Data Model for all cognitive assessment data, adherence to DoD Risk Management Framework with DoD IL5 cloud capability, HIPAA compliance with Role-Based Access Control, and administrative tools for managing approved assessment tools, data sources, and business rules.

software installation required.
MVC-2: UNSUPERVISED SELF-ADMINISTRATION: The solution MUST be designed for and proven to support unsupervised, self-administered testing by the Service ember, eliminating the need for a trained proctor.
MVC-3: PROVEN ENTERPRISE SCALABILITY: The solution MUST be a currently fielded, "battle-tested" platform with documented proof of its ability to support enterprise-level user loads (minimum 35,000 simultaneous sessions).
3.2 FUNCTIONAL REQUIREMENTS FOR FULL OPERATIONAL CAPABILITY (FOC)
The following functional requirements define the capabilities for the enduring, enterprise-wide solution to be built upon the initial MVC foundation.
FOC-1: Foundational System of Record.
FOC-1.1: The capability MUST create and maintain a unique, longitudinal cognitive health record for each individual ervice member, preserved according to NARA-approved schedules.
FOC-1.2: The capability MUST support the establishment and maintenance of an individual cognitive baseline, against which all future assessments for that individual are compared.
FOC-2: Comprehensive Assessment Platform.
FOC-2.1: The capability MUST support the administration of baseline cognitive tests and enable comparative post-injury tests to detect changes in cognitive function.
FOC-2.2: The capability MUST include assessments for the following core cognitive domains: Attention/Concentration, Memory, Processing Speed, and Executive Function.
FOC-2.3: The capability MUST provide a library of customizable cognitive tasks that allow authorized personnel to create and deploy tailored assessment batteries.
FOC-3: Data, Integration, and Analytics.
FOC-3.1: The capability MUST provide a modern, standards-based API to allow for secure data exchange with DoW enterprise systems, including MHS GENESIS, ILER, and authorized readiness platforms.
FOC-3.2: The capability MUST provide the ability to ingest historical data from the legacy ANAM system.
FOC-3.3: The capability MUST provide role-based dashboards and
CUI//PROCURE
reporting tools for clinicians, commanders, and senior leaders.
FOC-3.4: The capability MUST enforce a Common Data Model (CDM) for all cognitive assessment data.
FOC-4: Security, Privacy, and Governance.
FOC-4.1: The capability MUST adhere to the DoW Risk Management Framework (RMF) and be capable of operating in a DoW IL5 cloud environment.
FOC-4.2: The capability MUST adhere to HIPAA rules and be governed by a strict Role-Based Access Control (RBAC) framework.
FOC-4.3: The capability MUST provide tools for administrators to manage approved assessment tools, data sources, and business rules.
