About this file
This document is a Subsurface Utility Engineering (SUE) Test Hole Report for the Lake Nighthorse Road rehabilitation project, prepared by Goodbee & Associates for the Federal Highway Administration's Central Federal Lands Division. The investigation involved eight Quality Level A (QLA) test holes along a 0.8-mile road segment, focusing on utility mapping and identification. Key findings include three two-inch conduits owned by La Plata Electric, discovered at varying depths between 2.3 and 9.2 feet, with no fiber optic lines definitively located. Some test holes could not fully expose target utilities due to challenging ground conditions and deep electronic locate depths.
The report follows ASCE standard 38-22 for utility investigation, utilizing geophysical equipment and vacuum excavation techniques. Utility owners contacted included La Plata Electric, Fast Track Communications, and the Bureau of Reclamation (BOR). The investigation was conducted in October 2024, with recommendations for contractors to exercise caution, contact Colorado 811 before excavation, and continue coordination with utility owners, particularly regarding potential undetermined fiber optic lines. The project's survey used Colorado State Plane South Zone coordinates with NAD83 datum, and the test holes were surveyed with high precision (vertical accuracy of 0.1 ft, horizontal accuracy of 0.2 ft).

Introduction
The purpose of this Test Hole Report is to convey the process and findings of the Subsurface
Utility Engineering (SUE) QLA test hole excavation efforts. This report contains information about the utility investigation that may not otherwise be conveyed in the SUE deliverables, assists the end user in understanding the existing utilities and risks, provides recommendations to address data deficiencies, and complements the SUE base file. Utilities are depicted in the SUE base file in accordance with their achieved quality levels as defined in the American Society of
Civil Engineers, Utility Engineering & Surveying Institute, Construction Institute
(ASCE/UESI/CI) ASCE Standard 38-22, Standard Guideline for Investigating and Documenting
Existing Utilities.
This Test Hole Report and SUE investigation were performed for WSP (Owner) under a contract agreement with the Federal Highway Administration, Central Federal Lands Division (Client) to support the design prime contract between the Client and Owner. The purpose of this project is to rehabilitate the Lake Nighthorse Road.
The users of this data are reminded that this information is for design purposes only. Existing utility locations do not supersede Colorado 811 demarcations of underground utilities or relieve the contractor of the legal requirement to contact Colorado 811 prior to excavation.
Disclaimer and Certification
Existing utilities are depicted in this report and in accordance with the achieved Quality Levels for each utility as defined in ASCE Standard 38-22, Standard Guideline for Investigating and
Documenting Existing Utilities and as required by C.R.S. 9-1.5-101 et. Seq.
As it applies to the depiction of existing underground utility information, the stamp and seal on this report reflect only that depiction of the existing underground utility meets the noted ASCE
38 quality levels. Goodbee & Associates (Goodbee) will not be held responsible for the depiction of any changes in field conditions beyond the geophysical field investigations completed October 2024. Utilities may have been installed, removed, or adjusted after the field work was completed.
Reliance upon these data for risk management purposes during bidding does not relieve the excavator or utility owner from following all applicable utility damage prevention statutes, policies and/or procedures during excavation. It is important that the contractor investigates and understands the scope of work between the project owner and their engineer regarding the scope and the limits of the utility investigations leading to these utility depictions.
This Test Hole report for the Lake Nighthorse project was prepared under my direct supervision in accordance with ASCE standard 38-22 and scope of work.

SUE Field Processes and Equipment
Goodbee was scoped to complete SUE QLD and QLA investigations. SUE QLB geophysical investigations were beyond our scope of work. Geophysical instruments, such as multifrequency
Radio Detection equipment, were used prior to test hole excavation to better identify the horizontal location of utility lines before digging.
Test Hole Survey
The provided Control was checked by Goodbee using a Trimble R12i GPS receiver utilizing
Trimble RTX correction services and a Trimble TSC-5 Data Collector. Designating marks were tied to the project survey datum with an accuracy of 0.2 ft(60mm) horizontal.
QLA test holes were surveyed by Goodbee using a Trimble S5 Robotic Total Station. SUE elevation data were tied to the project survey datum with an accuracy of 0.1 ft (30 mm) vertical and to 0.2 ft (60 mm) horizontal.
Office Drafting Software
OpenRoads Designer was used to develop the 2D utility base file.
SUE Field Investigations and Findings
Goodbee performed the geophysical investigation for this project in accordance with its standard of practice, which generally conforms to the ASCE standard 38-22 and aligns with acceptable industry standards.
Test Hole observations/investigation and survey field work was performed between 10/07/2024 and 10/08/2024. Goodbee will not be responsible for the depiction of any changes in field conditions beyond 10/08/2024, including utilities and appurtenances that were installed, removed, or adjusted after the field work was complete.
General/Field Conditions
During QLA Test Hole observations, active construction was underway at the main entrance to Lake Nighthorse. This construction did not impede any of Goodbee’s efforts.
QLA Test Hole Findings
Eight test holes were included in the project scope of work. 8 test holes were completed and are summarized in Appendix A – SUE QLA Investigation (Test Hole Report).
The following summaries are provided to draw attention to special conditions such as discrepancies between field findings and records, unusual utilities, and any subsurface conditions that hindered the ability to perform the test hole excavation.
La Plata Electric o TH001- TH004, and TH006 - Three two-inch conduits were observed in these test holes.
All three conduits were confirmed to be owned by La Plata Electric.
Target utility not found in test hole o TH008 – Target utility was not found due to ground conditions. Electronic depth suggests a range of 15 feet or greater.
o TH007 – Target utility was not found due to ground conditions. Electronic depth suggests a range of 20 feet or greater o TH005 – Target utility was not found due to equipment restraints. Utility Marking tape was found near expected depth and the red plastic marking tape was consistent with the findings in TH001-TH004 and TH006.
Fiber Lines o No fiber optic lines/features were observed in the test holes or marked by CO 811 paint/marks. 811 paint was present for electric in the area, but no signs of fiber were present on site.
o Fiber line locations were not confirmed during SUE QLA investigations. It is assumed that two fiber owners may be present in the project area: BOR and FastTrack.
BOR noted they have an active fiber line, but no surface features (pedestals, handholes, or paint markings) were found in the field. BOR was not able to provide records of their facilities.
FastTrack provided records of their facilities and a representative from
FastTrack indicated that these facilities are likely not in use. FastTrack has elected not to further investigate, will not be relocating, and understands that future work could impact their facilities. See Appendix B for record data.
Conclusion
Reliance upon these data for risk management purposes during bidding does not relieve the excavator or utility owner from following all applicable utility damage prevention statutes, policies, and/or procedures during excavation. It is important that the contractor investigates and understands the scope of work between the project owner(s) and their engineer regarding the scope and limits of the utility investigations leading to these utility depictions.
Utility attribute information such as utility owner name, pipe size, and material are based on information gathered from available records, from discussion with utility owners, or as directly observed during Quality Level A (QLA) test hole excavation. In some cases, record information may be obsolete or inaccurate.
It is recommended that the owner, designer, and/or contractor:
• Confirm if new utility facilities have been installed since the SUE field data investigations.
• For utilities document in records but not detected during field work (QLD), it is recommended that the excavator proceeds with an abundance of caution and takes proper measures to notify utility owners through Colorado 811 prior to commencement of work.
• The contractor should follow C.R.S. 9-1.5-101 et. Seq.
• Contractor may encounter fiber during construction. Continued coordination with
FastTrack and BOR are recommended to verify location of fiber facilities prior to excavation.
Reference
ASCE Standard Guidelines for Utility Data Acquisition - Quality Level Definitions
Quality level D (QLD): a value assigned to a utility segment or utility feature not visible at the ground surface whose estimated position is judged through utility records, information from others, from visual clues such as pavement cuts, obvious trenches, or existence of service. A
QLD data attribute is assigned to a utility segment or utility feature after review and compilation of existing records, oral recollections, one-call (Colorado 811) or “private-locate” markings, managed data repositories, context with other achieved utility quality levels, and/or other evidence of existence. QLD data is more uncertain than QLC, QLB, and QLA. QLD data is less uncertain than utilities documented without any utility quality level barring a professional's statement of fact to the contrary. Quality level C (QLC): “information obtained by surveying and plotting visible above-ground utility features and by using professional judgement in correlating this information to Quality Level D information.”
Quality level C (QLC): a value assigned to a utility segment not visible at the ground surface whose estimated position is judged through correlating utility records or similar evidence to utility features, visible aboveground and/or underground. The utility anchor point on the utility features shall be tied to the project survey datum with an accuracy of 0.2 ft (60 mm) horizontal.
A QLC value judgment is assigned to a utility segment by using visible utility features to approximate the position of a utility segment between or in proximity to the visible utility features and in context with other achieved utility quality levels. QLC only pertains to the underground utility segment(s), not the utility feature(s). QLC data is more certain than QLD and is more uncertain than QLB and QLA.
Quality level B (QLB): a value assigned to a utility segment or subsurface utility feature whose existence and horizontal position is based on geophysical methods combined with professional judgement and whose location is tied to the project survey datum. A QLB value is assigned to a utility segment when the following conditions are met:
(1) the utility segment was detected through the application of appropriate geophysical methods;
(2) the geophysical signal was judged to be reliable;
(3) the interpreted position was judged based on knowledge and use of geophysical science, utility design and installation practices, available records, visual features, and influence of site conditions; and
(4) the source designation has been tied to the project survey datum with an accuracy of 0.2 ft
(60 mm) horizontally. QLB is more uncertain than QLA and more certain than QLC or QLD.
Quality level A (QLA): a value assigned to that portion (x-, y-, and z-geometry) of a utility segment or subsurface utility feature that is directly exposed and measured and whose location and dimensions are tied to the project survey datum. The utility segment or subsurface utility feature shall be tied to the project survey datum with an accuracy of 0.1 ft (30 mm) vertical and to 0.2 ft (60 mm) horizontal for the measurements of the outside limits of the utility feature or utility segment that is exposed. Other measurable, observable, and judged utility attributes are also recorded. If obtained by means of a test hole observation, a verification effort is made, and professional judgment is used to assert that the exposed infrastructure is indeed the sought target.
The assignment of QLA conveys the lowest level of relative (nonquantifiable) uncertainty of measurable and judged attributes and location. QLA is more certain than QLB, QLC, or QLD.
