This Task Assignment authorizes Golder to conduct supplemental site assessment activities to support an Interim Source Removal (ISR) Work Plan in accordance with the May 28, 2021, Scope of Work, Cost Estimate, and Bid Documentation.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Soil Sampling and MAGS Well/VMP Installation Golder will install ten soil borings and six modified active gas sampling (MAGS) wells. The six new MAGS wells and five existing MAGS wells will be used as vacuum monitoring points (VMPs). Prior to their installation, a utility locate will be conducted. Golder requested bids for the utility locate from GeoTek, Blood Hound, and McKim & Creed. GeoTek was selected as the lowest responsive and responsible bidder. Golder will collect soil samples at three depth intervals from the ten soil boring locations and four of the MAGS well locations (42 total samples) and analyzed for chlorinated volatile organic compounds (VOCs). Samples will also be collected using 5-gram Encore samplers for possible Synthetic Precipitation Leaching Procedure (SPLP) testing. Investigation-derived waste (IDW) will be temporarily stored in 55-gallon drums. IDW transport and disposal is covered under Subtask 3.

Subtask 3: Annual Groundwater Monitoring Golder will collect groundwater samples from monitoring wells MW001-MW013. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for VOCs. IDW will be temporarily stored in 55-gallon drums. Golder requested bids for IDW drum transport and disposal from Clark, Erwin, and Capitol Environmental. Erwin was selected as the lowest responsive and responsible bidder.

Subtask 4: MAGS Investigation Golder will conduct MAGS testing using the eleven MAGS wells at the site. Photoionization meters and Draeger colorimetric detector tubes will be used to monitor VOCs in the field. One

RE: COST PROPOSAL FOR A SUPLEMENTAL ASSESSMENT AND INTERIM SOURCE REMOVAL

WORK PLAN

SWIFT CLEANERS BEACHES SITE

1029 NORTH 3RD STREET, JACKSONVILLE BEACH, FLORIDA

DEP FACILITY ID NUMBER: ERIC_4583

PROPOSED TASK ASSIGNMENT: DC033B

Dear Mr. Grubbs:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal to prepare a supplemental assessment report and interim source removal (ISR) Work Plan including soil sampling and modified active gas sampling (MAGS) well installation, annual groundwater sampling, and MAGS investigation for the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Waste Site Cleanup Section requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.

SUBTASK 2 – SOIL SAMPLING AND MAGS WELL/VMP INSTALLATION

Golder will contact Sunshine 811 to initiate a utility locate for underground utilities and use a private utility locator to complete an underground utility locate where the proposed ten soil borings and six MAGS wells are located on the attached Figures 4 and 5. Two of the proposed MAGS wells are located adjacent to SB-1A and SB-4A soil boring resampling locations. Golder will core through the concrete surfaces inside and outside of the building at the selected/accessible locations and use a stainless-steel hand auger to collect soil samples to the top of the water table (approximately 5 feet below ground surface [bgs]). A total of ten soil borings and four borings for MAGS wells will be installed within the vadose zone to collect soil samples at 0.5, 2.0, and 3.0 feet bgs (42 total soil samples)

Mr. Ben Grubbs CX21-467710

Florida Department of Environmental Protection May 28, 2021 for laboratory testing, at each soil boring location and submitted to a fixed-based laboratory for chlorinated volatile organic compounds (VOCs) using EPA Method 8260B. In addition, forty-two soil samples will be collected using an Encore 25-gram sampler at each sample location interval for possible Synthetic Precipitation Leaching Procedure (SPLP) testing, if necessary, based on the evaluation of the total chlorinated VOC results exceeding the leachability soil cleanup target levels. The cost for the SPLP extraction and chlorinated VOC analysis is not included in this proposal and a Work Change Directive will be completed if this testing is necessary after consulting with the FDEP.

There are six proposed MAGS well locations that will be installed along with the five existing MAGS wells (MAG001 through MAG005) that will be used as vacuum monitoring point (VMP) locations during our testing. The five existing MAGS/VMP locations and six proposed MAGS well locations are shown on Figure 5. If any of the five existing MAGS/VMP locations are not accessible during the well installation activities, Golder will install another well at that location to the same construction as documented later in this paragraph. The six proposed MAGS wells will be screened from 0.5 foot bgs to a depth approximate 1.0 foot above the water table which is estimated to be approximately 3.5 feet bgs (bottom of the MAGS well). The MAGS wells will be constructed of 2-inch diameter Schedule 40 PVC, 0.010-inch machine slotted screens. After the PVC well pipe has been installed, a filter pack consisting of 20/30 grade silica sand will be placed within the annular space and brought up to a level above the top of the slotted section of screen, followed by the placement of a layer of bentonite grout to the surface to create a seal. The wells will be flush mounted with the surrounding surfaces and protected using a locking cap and steel cover to avoid damage.

Soil samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018). The soil will be collected and placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program (NELAP)-certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B.

Golder estimates two days for a two-person field crew to complete soil sampling and MAGS wells/VMP installation.

Soil cuttings from the hand auger borings will be placed in 55-gallon drums, appropriately labeled as IDW, and secured onsite to await waste characterization, profiling, and disposal acceptance. For cost estimating purposes, Golder assumes characterization and disposal of two drums of non-hazardous IDW soil. The drill cuttings will be disposed of in accordance with state and federal regulations.

SUBTASK 3 – ANNUAL GROUNDWATER MONITORING

Golder will collect groundwater samples from the following 13 wells: four shallow zone monitoring wells (MW001, MW002, MW003, and MW005); six intermediate zone monitoring wells (MW004, MW007, MW008, MW009, MW011, and MW012); and three deep zone monitoring wells (MW006, MW010, and MW013). Tables and Figures with monitoring well construction and previous groundwater analytical data, from an E&E report dated April 2020, are provided in Attachment A. The approximate monitoring well locations are presented on Figure 3.

Prior to groundwater sampling, the water level depth will be measured at the wells to be sampled to determine groundwater elevations and groundwater flow direction in the shallow, intermediate and deep surficial aquifer.

Groundwater samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP-SOP-001/01) dated January 2017 approved April 16, 2018. Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures. During the sampling event, the locks and well caps will be inspected for damage or degradation and replaced, as necessary.

Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method No. 8260B. For quality assurance purposes, one trip blank will accompany each sample container for laboratory analysis of chlorinated VOCs using EPA Method 8260B. Upon receipt of the laboratory analytical results, Golder will evaluate the groundwater concentrations to determine if off site notification is required.

The purge water will be containerized in a 55-gallon steel drums and stored at the site pending waste characterization and disposal. A sample of the IDW purge water will be collected and analyzed for total VOCs using EPA Method 8260. For cost estimating purposes, Golder assumes characterization and disposal of two drums of non-hazardous IDW purge water. The four drums of IDW, two drums from well purging activities and two drums of soil generated during Subtask 2, will be picked up and transported to a disposal facility by a subcontractor.

SUBTASK 4 – MAGS INVESTIGATION

Once the MAGS wells have been installed, Golder will commence with the MAGS investigation to evaluate the nature and extent of chlorinated VOC impacts in the vadose zone soils. The MAGS investigation results will be used to Identify any source areas located near the boiler room and dry cleaning machine area.

A MAGS assessment involves fitting a regenerative blower to a temporary soil vapor extraction well and, as soil vapor is extracted, it is analyzed on-site using an OVA-PID and/or Draeger colorimetric detector tubes. This methodology allows soil vapor gas to be extracted at a relatively high rate to evaluate soil impacts within the radius of influence (ROI) of the well screen where vacuum is being applied. Soil gas vapor is extracted using a portable regenerative blower (AMETEK ROTRON model EN505AX58ML, 2.0 hp, single phase). The blower is capable of a flow rate of 80 scfm at a vacuum of 50 inches of water (inches H20) under ideal conditions. Power is supplied to the blower via a portable generator. The connection between the wellhead and blower is made using 2-inch diameter vacuum rated flexible PVC tubing and Schedule 40 PVC pipe. Vacuum gauges, a flowmeter, filter, and sample ports are installed in-line.

The MAGS wells are positioned to cover previously reported areas of impact (near former drycleaning machine location and outside the rear door) and identify additional potential sources within the expected radius of influence of the MAGS wells. Based on the previous soil vapor extraction (SVE) system operated at this facility, a ROI in excess of 20 feet was obtained from the VES wells; therefore, a preliminary 20-foot ROI has been assumed for MAGS well spacing. While extracting air from a given extraction point, vacuum measurements will be recorded at other nearby vapor extraction wells (MAGS wells) for evaluating the ROI for given vacuum conditions.

Each of the proposed MAGS wells will be tested using the historical data collected from operating the previous VES system in the vadose zone at this site to obtain the effective field ROI, soil permeability, hydraulic conductivity, pore volume, purge time, and sampling frequency. Based on these calculations, the time that vapor is extracted at each

MAGS well will be adjusted accordingly. An OVA-PID will be used to monitor off-gas (i.e., PCE) from the MAGS sampling point. Field analysis of soil vapor for PCE will be conducted using Draeger colorimetric detector tubes.

The field OVA-PID and detector tubes will be used to monitor extracted vapor at the primary wellheads for VOC concentrations, and the measurements will be graphed as concentrations versus time. The actual ROI will then be estimated mathematically for those times at which VOC concentrations were the greatest. This information will be used to identify the general locations of impacted soils using the data from each extraction point.

Near the end of the MAGS extraction at each location, one soil vapor sample will be collected in a tedlar bag for subsequent laboratory analysis to provide confirmation of the field OVA-PID results and to provide a general assessment of the types and concentrations of remaining chlorinated solvents in soil. This work plan includes a total of six soil vapor samples that will be submitted for fixed-base laboratory analyses using EPA Method TO-15.

A standard turn-around time will be used for analyzing these samples. Golder estimates that the MAGS assessment will take one day to complete.

SUBTASK 5 – SUPPLEMENTAL ASSESSMENT REPORTING

The supplemental assessment report will include a description of sampling procedures, evaluation of analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the soil and groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will update the FDEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the FDEP data checker on the completed tables; and submit the updated tables and error report under this subtask. Additionally, Golder will forward Automated Data Processing Tool (ADaPT) data deliverables provided by the laboratory for the groundwater sampling event.

SUBTASK 6 – INTERIM SOUCE REMOVAL WORK PLAN

Following evaluation of the Supplemental Assessment Report soil and soil vapor testing results, Golder will prepare an ISR Work Plan to complete a limited soil excavation and/or install an SVE system to address the source of contamination in the soil vadose zone. The ISR Work Plan will include the following:

The horizontal extent of the chlorinated VOC impacted soil both the site and property to the east of the subject site, including a delineation of non-hazardous waste soil from the supplemental assessment report.

The proposed site preparation activities, including coordination with property owners, utility location services, tree removal services, a professional land survey, erosion control measures, and documentation of pre-excavation and/or SVE System installation site conditions.

The proposed soil excavation and/or monthly maintenance procedures for the SVE system.

The proposed backfill and site restoration activities, if necessary.

Health and safety considerations, including recommended personal protective equipment and dust control measures.

An estimated cost and schedule to implement the ISR Work Plan.

HEALTH AND SAFETY

Golder will prepare a site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots.

COST AND SCHEDULE

The estimated cost to complete the monitoring is $41,849.49. Golder will tentatively schedule to begin the soil sampling and MAGS well installation in July 2021. Golder anticipates that a two person field crew will take two days for soil sampling and MAGS well installation, one day to complete the annual groundwater sampling, and one day to complete MAGS well sampling. The LOE/CEF cost sheets and subcontractor bids are included in Attachment B.
