RE: COST PROPOSAL FOR INTERIM SOURCE REMOVAL WORK PLAN
JAY’S PROFESSIONAL DRY CLEANERS AND LAUNDRY
10025 SAN JOSE BOULEVARD
JACKSONVILLE, DUVAL COUNTY, FLORIDA
FDEP FACILITY ID NUMBER: ERIC_4521
PROPOSED TASK ASSIGNMENT: DC008L
Dear Mr. Grubbs:
Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal to develop an Interim Source Removal (ISR) Work Plan for the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.
SUBTASK 1 – PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP Waste Cleanup Section requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.
SUBTASK 2 – INTERIM SOURCE REMOVAL WORK PLAN
Golder performed soil sampling activities in October 2019 to complete the delineation of chlorinated volatile organic compound (VOC) contamination in onsite soils. During previous soil sampling activities, Golder identified tetrachloroethene (PCE) in soil at concentrations above the Soil Cleanup Target Level (SCTL) for leachability, based on groundwater criteria, at multiple locations. Subsequent VOC analysis using the Synthetic Precipitation Leaching
Procedure reported PCE concentrations above the Groundwater Cleanup Target Level. The October 2019
Mr. Ben Grubbs FDEPDC008 P13-82530
Florida Department of Environmental Protection December 26, 2019 analytical results indicated that the horizontal extent of chlorinated VOC contamination at the site has been delineated. Golder recommended developing an ISR Work Plan to excavate the soil with chlorinated VOC concentrations present above the SCTL for leachability, and the FDEP agreed. The proposed excavation layout, based on the soil sampling results, is shown on the attached Figure 4.
The ISR Work Plan will include the following:
A summary of the previous site assessment activities completed at the site.
The horizontal and vertical extent of the proposed ISR activities.
The proposed pre-excavation activities, including coordination with the property owner and nearby tenants, utility location services, a well abandonment or protection plan, an asbestos survey, a demolition plan for non-load bearing walls within the proposed excavation area, a professional land survey, erosion control measures, a site security plan, permitting, and documentation of pre-excavation site conditions.
Structural engineering plans (prepared Baker Design Build), including the following: documentation of load-bearing walls and columns, documentation showing slab removal extents and depth of excavation, and soil replacement and slab repair specifications.
The proposed soil excavation and waste management procedures.
The proposed bio and/or chemical amendment and dosage to be added to the base of the excavation area prior to backfill, including an Underground Injection Control (UIC) Notice and a UIC Monitoring Plan.
The proposed backfill and site restoration activities, including replacement of interior walls, plumbing, and electrical, as necessary.
Health and safety considerations, including recommended personal protective equipment and dust/vapor control measures.
A Remedial Action Plan Checklist.
An estimated cost and schedule to implement the ISR Work Plan.
COST AND SCHEDULE
The estimated cost to complete the scope of work is $16,805.97. The LOE/CEF cost sheets are included in
Attachment A. The structural engineering services quote from Baker Design Build is included in Attachment B. The following table summarizes the proposed costs and deliverable schedule.

DATE: December 18, 2019
TO: Mr. Matt Crews, P.E.
Golder Associates Inc.
9428 Baymeadows Road, Suite 400 Jacksonville, FL. 32256
PROJECT: Former Jays Cleaners 10025 San Jose Blvd., Jacksonville Florida
PROJECT INFORMATION
Based on an email sent on December 13, 2019 , it’s understood that an existing concrete block building with stem wall shallow footings has contaminated soil beneath the slab area located at the above referenced address. It is estimated that about 30% of the building is sitting on contaminated soil which will be required to be removed. Our services will include documentation of the load-bearing walls and columns and documentation showing removal slab extents and anticipated 2 to 4 feet of soil depth removal (to be Determined). The walls/columns will also need to be stabilized and support provided through the use of helical anchors. BDB will provide soil replacement and slab repair specifications. An initial site visit is required prior to the start of the engineering plans and is included within the scope of this proposal. Any additional site visits requested will be billed at an hourly rate.
