This Task Assignment authorizes Golder to conduct well redevelopment and groundwater monitoring activities in accordance with the April 21, 2021, Scope of Work, Cost Estimate, and Bid Documentation.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Monitoring and Injection Well Redevelopment Golder will redevelop monitoring wells MW034-MW036 and MW038-MW041 and injection wells IW002-IW008. Investigation-derived waste (IDW) will be temporarily stored in 55-gallon drums until waste characterization is completed. IDW disposal will be covered under Subtask 3.
Subtask 3: Annual Groundwater Monitoring and IDW Treatment Golder will collect groundwater samples from five shallow zone monitoring wells (MW001, MW16, MW034, MW038, and MW040); nine intermediate zone monitoring wells (MW011, MW023, MW029, MW018, MW035, MW037, MW039, MW041, and MW019); seventeen injection wells (IW002, IW003, IW004, IW007, IW008, IW011, IW012, IW013, IW005, IW006, IW015, IW017, IW018, IW019, IW021, IW022, and IW023), and five deep zone monitoring wells (MW015, MW024, MW-27, MW030, and MW036). Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Groundwater samples from MW029, MW034, MW035, IW021, IW022, and IW023 will also be analyzed for total organic carbon. Groundwater samples from MW029, MW034, and MW035 will also be analyzed for total dissolved solids, color, and odor.
IDW will be temporarily stored in 55-gallon drums until waste characterization is completed. An IDW sample will be collected and analyzed for VOCs. Golder requested bids for IDW transport

April 21, 2021 Proposal No. CX21-460854
Mr. Benjamin Grubbs Florida Department of Environmental Protection 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400
RE: COST PROPOSAL FOR DIXIE CLEANERS
WELL DEVELOPMENT AND ANNUAL GROUNDWATER MONITORING
1931 BLANDING BOULEARD, JACKSONVILLE, FLORIDA
DEP FACILITY ID NUMBER: ERIC_4531
PROPOSED TASK ASSIGNMENT: DC014P
Dear Mr. Grubbs:
Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal for redevelopment of selected monitoring wells impacted from the building demolition and completing the annual groundwater monitoring event at the Dixie Cleaners site located at 1931 Blanding Boulevard in Jacksonville, Duval County, Florida. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services.
The general scope of work was previously discussed during the January site summary meeting with the FDEP. The proposed scope of services is described below.
SUBTASK 1 – PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.
SUBTASK 2 – MONITORING AND INJECTION WELL DEVELOPMENT
Golder proposes to mobilize a two-person sampling crew to the site to redevelop seven monitoring wells MW034, MW035, MW036, MW038, MW039, MW040, and MW041 and seven injection wells IW002, IW003, IW004, IW005, IW006, IW007, and IW008, (Figures 7 and 8) that were impacted by the building demolition activities will be redeveloped before completing the annual groundwater sampling activities. Well construction information for the
Mr. Benjamin Grubbs Proposal No. CX21-460854
Florida Department of Environmental Protection April 21, 2021 affected monitoring wells is presented on Table 1. The property owner has not informed Golder on the locations of proposed features on the property during the redevelopment phase; therefore, Golder has not included the surface completions for the above-referenced monitoring/injection wells during this phase of work.
The development water will be containerized in a 55-gallon steel drums and stored at the site pending waste characterization and disposal. A sample of the investigative derived waste (IDW) development water will be collected and analyzed for chlorinated volatile organic compounds (VOCs) using EPA Method 8260B. For cost estimating purposes, Golder assumes characterization and disposal of six drums of non-hazardous IDW waste water.
SUBTASK 3 – ANNUAL GROUNDWATER MONITORING AND IDW TREATMENT
Golder will collect groundwater samples from the following 36 wells: five shallow zone monitoring wells (MW001, MW16, MW034, MW038, and MW040); nine intermediate zone monitoring wells (MW011, MW023, MW029, MW018, MW035, MW037, MW039, MW041, and MW019); seventeen injection wells (IW002, IW003, IW004, IW007, IW008, IW011, IW012, IW013, IW005, IW006, IW015, IW017, IW018, IW019, IW021, IW022, and IW023), and five deep zone monitoring wells (MW015, MW024, MW-27, MW030, and MW036). Figures 12 through 14 from the most recent groundwater monitoring report, which show the well locations and most recent VOC results for the shallow, intermediate, and deep zones of the surficial aquifer, are attached for reference.
Prior to groundwater sampling, the water level depth will be measured at the wells to be sampled to determine groundwater elevations. Groundwater samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP-SOP-001/01) dated January 2017 approved April 16, 2018.
Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures. During the sampling event, the locks and well caps will be inspected for damage or degradation and replaced, as necessary.
Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method No. 8260B. For quality assurance purposes, one trip blank will accompany each sample container for laboratory analysis of chlorinated VOCs using EPA Method 8260B. In addition, groundwater samples from six wells (MW029, MW034, MW035, IW021, IW022, and IW023) will be analyzed for total organic carbon using EPA Method SM 5310B. Groundwater samples from three of the monitoring wells (MW029, MW034, and MW035) will also be analyzed for CHITOREM® Underground Injection Control parameters including total dissolved solids using EPA Method 160.1, color, and odor. Upon receipt of the laboratory analytical results, Golder will evaluate the groundwater concentrations to determine if off site notification is required.
The purge water will be containerized in a 55-gallon steel drums and stored at the site pending waste characterization and disposal. A sample of the IDW purge water will be collected and analyzed for total VOCs using EPA Method 8260. For cost estimating purposes, Golder assumes characterization and disposal of three drums of non-hazardous IDW waste water. The six drums of IDW from well development activities during Subtask 2, will also be picked up for disposal by the subcontractor.
SUBTASK 4 – COVID-19 COSTS
Providing a second truck for use by a two-person field crew during a one day mobilization associated with Subtask 2 and three days of mobilization associated with Subtask 3 to provide protection from Covid-19.
SUBTASK 5 – REPORTING
The annual groundwater monitoring report will include a description of sampling procedures, evaluation of analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will update the FDEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the FDEP data checker on the completed tables; and submit the updated tables and error report under this subtask. Additionally, Golder will forward Automated Data Processing Tool (ADaPT) data deliverables provided by the laboratory for the groundwater sampling event.
HEALTH AND SAFETY
Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.
COST AND SCHEDULE
The estimated cost to complete the monitoring is $22,885.32. Golder will tentatively schedule the well development work for May 2021 and the groundwater sampling one to two weeks later. Golder anticipates that development of monitoring/injection wells will take one day and the water level gauging and groundwater sample collection will take three days to complete. The LOE/CEF cost sheets and subcontractor bids are included in Attachment A. The following table summarizes the proposed costs and deliverable schedule.

AEL’s testing laboratories are accredited by NELAP by the Florida Department of Health. It is the client’s responsibility to ensure AEL is aware of any testing requiring accreditation. AEL makes no claims or guarantees that reported MDLs, PQLs, and/or MRLs will meet a Client’s requirements, be they regulatory, commercial, industrial, or otherwise. It is the complete and full responsibility of the Client to determine if AEL MDLs, PQLs, and/or MRLs will satisfy a Clients needs.
7. WARRANTIES:
AEL performs services and hence its work and reports are not governed by the Uniform Commercial Code. Except as stated in paragraph 6, AEL disclaims all warranties, express or implied, including any warranties of merchantability or fitness for a particular purpose. AEL shall have no liability for incidental or consequential damages of any nature whatsoever.
8. Cancellation of Testing:
AEL begins testing upon receipt of samples from the Client. If for any reason the testing is interrupted by the Client, the completed portion of the work, including (but not limited to) supplies, materials, labor, and equipment utilization shall be billed to the Client and be payable within the agreed upon payment terms. In such circumstances where AEL agrees to place samples on hold, AEL may bill the Client stand-by charges, up to the full cost of established testing rates, until the testing resumes or is terminated by the client. If the testing is terminated by the client, AEL may impose a cancellation fee of no less than 10% of the unbilled portion of the project.
9. PROGRAM DELAYS:
AEL shall not be liable for any failure or delay in performance which is caused in whole or in part by acts of God (fire, flood, earthquakes, etc.), strikes or other labor disturbances, shut-downs, equipment breakdowns, unforeseen engineering problems, fuel shortages, Government priorities, or any other cause beyond the control of AEL.
10. ACCEPTANCE OF TEST REPORTS:
AEL reports apply only to the specific samples tested under stated test conditions and test results are not necessarily indicative of the qualities of apparently identical or similar test or operating conditions. AEL shall have no liability for any deductions, inferences or generalizations drawn by the client or others from AEL reports. If the Client requests verification of any part of the test report, AEL shall be notified within thirty (30) days of submission of the report to the Client. Failure to notify within this thirty
(30) day period acknowledges acceptance of the report. Should additional work be required for verification purposes, AEL shall be entitled to the reasonable value of the additional work involved providing the original findings are verified.
Payment for any test report or other AEL invoice shall not be contingent upon acceptance and/or approval of a third party.
11. SAMPLE/DATA RETENTION
Samples will be destroyed thirty (30) days after the date of the final report, unless the client indicates otherwise in writing and prepays before the expiration of said thirty (30) day period the entire cost of any storing, packaging and shipping the sample(s) by AEL. AEL shall have no obligation to retain its test reports or related data and documents beyond its normal retention periods.
12. SHIPPING/TRANSPORTATION
The Client shall be responsible for the transportation and any associated shipping costs of Client’s property to and from AEL’s laboratories.
