HW561 DC004P

This Task Assignment authorizes Golder to conduct groundwater monitoring activities in accordance with the May 3, 2021, Scope of Work, Cost Estimate, and Bid Documentation.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Monitoring and IDW Treatment Golder will collect groundwater samples from six monitoring wells (MW005, MW006, MW011, MW017, MW024, and MW025). Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum until waste characterization is completed. An IDW sample will be collected and analyzed for VOCs. Golder requested bids for IDW transport and disposal from Clark Environmental, Erwin Remediation, and Capitol Environmental. Erwin Remediation was selected as the lowest responsive and responsible bidder.

Subtask 3: COVID-19 Costs Costs related to the use of an extra vehicle, so that Golder can maintain social distancing during the COVID-19 pandemic, are covered under this subtask for invoicing purposes.

Subtask 4: Reporting Golder will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $6,595.42 with 45 labor hours.

May 3, 2021 Proposal No. CX21-465590

Mr. Ben Grubbs Florida Department of Environmental Protection Division of Waste Management Drycleaning Solvent Cleanup Program 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400

RE: COST PROPOSAL FOR THE FORMER JET CLEANERS FACILITY

ANNUAL GROUNDWATER MONITORING

1346 GANDY STREET

JACKSONVILLE, DUVAL COUNTY, FLORIDA

FACILITY ID NUMBER: ERIC_4540

PROPOSED TASK ASSIGNMENT: DC004P

Dear Mr. Grubbs:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for groundwater monitoring at the former Jet Cleaners site, located in Jacksonville, Duval County, Florida. This proposal includes a description of the proposed scope of services, level of effort (LOE) sheets for each subtask, and a Cost Estimate Form (CEF) that summarizes the costs of the proposed services. The proposed scope of services is described below. Recommendations Golder made as a result of the Soil, Soil Vapor, and Groundwater Sampling Letter Report, dated February 22, 2021, included the installation of two shallow wells and an Interim Source Removal Work Plan for implementing a soil vapor extraction system. Given that the groundwater plume appears to be stable, the DEP will consider these recommendations at a later date.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.

Mr. Ben Grubbs Proposal No. CX21-465590

Florida Department of Environmental Protection May 3, 2021

SUBTASK 2 – ANNUAL GROUNDWATER MONITORING AND IDW TREATMENT

Golder will collect groundwater samples from six monitoring wells (MW005, MW006, MW011, MW017, MW024, and MW025). The attached figures (Figures 2 through 6) from the 2020 annual groundwater monitoring report show the monitoring well locations, potentiometric maps, and chlorinated volatile organic compound (VOC) results for the shallow and intermediate zones of the surficial aquifer.

Prior to groundwater sampling, the depth to groundwater will be measured at accessible monitoring well locations (MW001, MW002, MW003, MW004, MW009, MW010, MW012, MW014, MW015, MW016, MW018, MW019, MW022, MW023, MW099) using an electronic water level indicator to determine groundwater elevations and flow direction. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced as necessary. Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP-SOP-001/01) dated January 2017 (effective April 16, 2018). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.

Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program-certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. For quality assurance purposes, one trip blank will accompany each sample cooler for laboratory analysis of chlorinated VOCs using EPA Method 8260B.

Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total VOCs using EPA Method 8260. If the waste characterization sample is acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.

Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, and laboratory chain-of-custody documentation.

SUBTASK 3 – COVID-19 COSTS

Providing a second truck for use by a two person field crew during a one day field effort associated with Subtask 2 to provide protection from Covid-19.

SUBTASK 4 – REPORTING

The Groundwater Monitoring Report will include a description of sampling procedures, evaluation of analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will update the FDEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error report. Additionally, Golder will forward Automated Data Processing Tool (ADaPT) data deliverables provided by the laboratory for each groundwater sampling event.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the monitoring is $6,595.42. Golder will tentatively schedule the annual sampling event for June 2021. The LOE/CEF cost sheets and subcontractor bids are included in Attachment A. The following table summarizes the proposed costs and deliverable schedule.
