RE: COST PROPOSAL FOR WELL ABANDONMENT

HARBOUR PLACE CLEANERS

13170 ATLANTIC BOULEVARD

JACKSONVILLE, DUVAL COUNTY, FLORIDA

FACILITY IDENTIFICATION NUMBER: ERIC_4538

PROPOSED TASK ASSIGNMENT: DC016M

Dear Mr. Grubbs:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal for well abandonment at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.

SUBTASK 2 – WELL ABANDONMENT AND LETTER REPORT

On July 16, 2020, the FDEP approved Golder’s recommendation that a Site Rehabilitation Completion Order (SRCO) be issued for the site. Once the SRCO is issued, Golder will abandon the thirteen existing monitoring wells (MW001 through MW013) in accordance with FDEP and St. Johns River Water Management District (SJRWMD) regulations. Seven Modified Active Gas Sampling (MAGS) wells (MAGS001 through MAGS007) were abandoned during the SVE system decommissioning completed in January 2012. Golder will use an in-house, Florida-licensed driller to abandon the monitoring wells.

The monitoring wells will have the steel manholes removed after abandonment activities are completed, and the ground surface will be restored to near its original condition. For the six wells (MW003, MW004, MW005, MW006, Mr. Ben Grubbs P13-82530

Florida Department of Environmental Protection October 15, 2020

MW007, and MW012) located in the asphalt drive area, the property owner recently repaved the parking and drive areas with asphalt and the concrete well pads were covered just leaving the steel manhole exposed slightly below the new asphalt surface grade (See attached photos). Golder believes the best way to address these well finishes, after grout is pumped into the well and returns to the surface, is to fill in the top of the well with concrete to the top of the existing grade, break off the metal ring, and use asphalt seal to cover the concrete surface blending it in with the surrounding asphalt. Well details and locations are included in the attached table and figure. After the field work is completed, the driller will prepare the appropriate well completion reports for submittal to the SJRWMD, updating the status of the wells at the site. The well abandonment field activities will be completed in two days and documented with photographs.

Golder will submit a letter report to FDEP documenting the well abandonment activities, which will include copies of the well completion reports and the photographs.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures.
