About this file
This is a Task Assignment Notification Form for an indefinite delivery contract between the Florida Department of Environmental Protection (FDEP) and Ecology and Environment, Inc. (E&E), a contracted environmental consulting firm. The task assignment, numbered DC004K under Contract HW553, pertains to groundwater monitoring services at the Blue Ribbon Cleaners site in Tallahassee, Leon County, Florida (Facility ID 37-9500612; ERIC_4859). The document was executed on November 22, 2019, with Ben Grubbs serving as the FDEP Contract Manager and Perry Kelso as the contractor representative.
The task assignment authorizes E&E to conduct annual groundwater monitoring activities comprising two subtasks: Project Management (calculated at 12.5% of direct labor costs) and Annual Groundwater Sampling and Report. The scope includes collection of groundwater samples from eight monitoring wells (MW001-MW008) using passive diffusion bags (PDBs), analysis for chlorinated volatile organic compounds (VOCs) via EPA Method 8260, and preparation of an Annual Groundwater Monitoring Report with professional engineering certification. The total fixed-price task assignment value is $7,949.96, encompassing 61.6 labor hours, laboratory analysis costs of $1,350.00, sample shipping costs of $40.00, equipment costs of $283.84, and travel costs of $194.00. The deliverable—the Annual Groundwater Monitoring Report—is due October 15, 2020, with task completion scheduled for December 31, 2020. Invoicing occurs upon submittal and FDEP approval of the final report, with no retainage applied.

HW553 DC004K
This Task Assignment authorizes E&E to conduct groundwater monitoring activities in accordance with the November 22, 2019, Scope of Work and Cost Estimate.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Annual Groundwater Sampling and Report E&E will collect groundwater samples from monitoring wells MW001-MW008. Prior to sampling, depth-to-water will be measured. The groundwater samples will be collected using passive diffusion bags (PDBs) and analyzed for chlorinated volatile organic compounds (VOCs).
E&E will submit the Annual Groundwater Monitoring Report to FDEP.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $7,949.96 with 61.6 labor hours.
E&E may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Re: 2020 Annual Groundwater Monitoring Cost Proposal for the Blue Ribbon Cleaners Site, Tallahassee, Leon County, Florida (ERIC_4859; Task Assignment HW553-
DC004K)
Dear Benjamin:
Ecology and Environment, Inc. (E & E) is submitting to the Florida Department of Environmental Protection (FDEP) this annual groundwater monitoring cost proposal package the above-referenced site.
This cost proposal package includes a monitoring schedule (Table 1), site plan (Figure 1), a proposed field work and reporting schedule for the completion of subtasks, and a cost estimate form / level of effort (Attached Separately). A brief summary of the scope of work is provided below:
Subtask 1: Project Management This subtask consists of project management hours only and is automatically calculated based upon 12.5% of the direct labor costs.
Subtask 2: Annual Groundwater Sampling and Report The annual groundwater sampling event will include installation of passive diffusive bags (PDBs) and subsequent collection of groundwater samples as outlined in Table 1 for laboratory analysis.
E & E personnel will mobilize to the site and install the PDBs into the screen intervals of the monitoring wells as indicated on Table 1. Following installation, and due to low aquifer permeability, the passive diffusive bags will be left for approximately one month prior to removal and collection of samples. E & E recommends using the previously acquired passive diffusive bag assemblies (hangers) as they were custom fitted for each well. Costs for passive diffusive bags were requested from ALS Life Sciences Division (ALS), (Attachment A).
The groundwater samples, a duplicate sample, and a trip blank will be delivered to the Advanced Environmental Laboratories Tallahassee, Florida, (AEL Tallahassee) facility and analyzed for volatile organic compounds using United States Environmental Protection Agency (USEPA) Method 8260. Well locations are provided on Figure 1.
Prior to the installation of the passive diffusion bags, depth-to-water will be gauged and recorded in the monitoring wells to determine groundwater flow direction(s) across the site and to ensure the passive diffusive bag is placed within the water table.
Mr. Benjamin Grubbs November 22, 2019
Upon receipt of the analytical data from AEL Tallahassee for the annual sampling event, E & E will prepare an Annual Groundwater Monitoring Report. The Annual Groundwater Monitoring Report will include a PG/PE certification, analytical results, groundwater contaminant concentration figures, groundwater elevation isopleth maps, summary of the sampling event, conclusions, and recommendations.
Upon FDEP approval of the Annual Groundwater Monitoring Report, an electronic copy of the report and updated GIS data tables will be submitted to FDEP via the FDEP FTP site or by e-mail.
Schedule The proposed project task schedule for completion of this work is as follows:
Place PDB Bags: July 15, 2020 Completion of Sampling: August 31, 2020 Completion of Annual Groundwater Monitoring Report: October 15, 2020 Task Assignment End Date: December 31, 2020
Cost The cost estimate to complete the above-described work plan/cost is provided in the associated Excel worksheet (Blue Ribbon Cost). E & E estimates the work will cost $7,949.96 to complete.
If you have any questions please give me (banders@ene.com) a call at (850) 523-0954.
Sincerely, ECOLOGY AND ENVIRONMENT, INC.
