SUMMARY OF CHANGES
May 6, 2011 Section C is amended as follows:
Section C.7.
“Nursing Home” language in CDI narrative and table is removed. Work is to be performed in hospitals only.
Section C.7.3
For HAI work, sustainability mode should be July 31, 2014.
Section C.7.2. Reduce Healthcare Acquired Conditions by 40% in Nursing Homes
The recruitment criteria will be based upon both the last two quarters of MDS 2.0 and the most recent available data of MDS 3.0. The NCC will be providing a list of overlapping homes between the two data sources for recruitment purposes only. For PR in particular, we expect the QIO to provide technical assistance to those NH who are in the >75% percentile of physical restraints in the state and who have a statewide physical restraint rate of >/=4%. It is possible that there are some states/facilities that will not be eligible to provide/receive direct technical assistance. However, there is considerable flexibility in what topics are covered in the Learning and Action Networks and it is highly likely that PrUs will be included in the HAC collaborative work occurring during Phase II.
Section C.7.2.B Specific Requirements: Reduce Healthcare Acquired Conditions by 40% in nursing homes: Pressure Ulcers and Physical Restraints (Phase I. First 18 months)
MDS 3.0 definition includes Stages 2, 3, and 4.
Section C.10.3.C.1, page 73 of 93, new note added to SOW paragraph (c) that reads:
c.
The ISA(s) shall report on his/her activities every six months in a semi-annual report deliverable. This deliverable shall be due on October 1 and April 1 of every year of this SOW. Each report will feature a diary of activities of the ISA(s) over the previous period (i.e., the 10/1/11 deliverable shall summarize work from 08/01/11 through 09/30/11, and the 4/1/12 deliverable shall summarize work from 10/1/11 through 03/31/12). The report shall follow a format prescribed by CMS and its coordinating center contractors. At a minimum, this report shall include a record of CRISP conference attendance, summary of Brand ambassador trainings, and summary of collaboration tool activities by the QIO. Summary of QIO Internet web presence activities and summary of use of social media by the QIO (once that social media is pre-approved by CMS in accordance with Attachment J-11 of this SOW.
Section C.7.3.B.2, revised to add:
“For QIOs in states in which a PSPC community team does not exist, the QIO will focus initially on developing a new community team with teams throughout the state. Teams will need to commit to the Expectations of Collaborative Teams outlined in J-12.”
Section C.7.3.B.3, revised to add:
The QIO shall select high-risk patients for the patient groups to maximize impact and significantly contribute to the national goal. The QIO shall work to recruit a total of 100 Medicare, Medicare Advantage and dual eligible beneficiaries to serve as the team’s Population of Focus (PoF) by October 31, 2011.
Section C.7.3.B.4 , revised to add:
The QIO shall recruit on a rolling basis so that teams have the benefit of participating in national collaborative learning sessions (date and locations to be determined). The QIO must seek to lead or support no fewer than five teams over the course of the 10th SOW for participating in the PSPC.
Section C.7.3.B.5, revised to add:
Data accessible to community teams includes: manual chart review EMR, /EHR data extraction, and retrospective drug utilization review by clinical pharmacist.
Section C.7.3.B.6, revised to add:
