About this file

This is a Conflict of Interest Questionnaire form required by the State of Texas for vendors conducting business with local governmental entities under Chapter 176 of the Local Government Code. The form applies to the Collin County Sheriff's Office prisoner transport services solicitation and must be completed by any vendor that meets specific conflict of interest criteria outlined in Section 176.006(a). Vendors are required to file the completed questionnaire with the records administrator of the local governmental entity no later than the seventh business day after the later of either the date the vendor begins contract discussions or submits a proposal, or the date the vendor becomes aware of a potential conflict of interest. The questionnaire requests disclosure of employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12-month period, any gifts with an aggregate value exceeding $100 provided to officers or their family members within the past 12 months, and any family relationships with local government officers.

Failure to file the questionnaire or knowingly providing false information constitutes a misdemeanor offense under Texas law. The form requires vendors to identify themselves, name any affected local government officer, describe the nature of any business or employment relationships, and disclose gifts or family relationships that may create conflicts of interest. Vendors must also indicate whether updates to previously filed questionnaires are being submitted if circumstances change. The questionnaire does not impose financial requirements or specific pricing terms but rather serves as a conflict-of-interest disclosure and compliance mechanism to ensure the integrity of the procurement process for Collin County's prisoner transport contract opportunity with a submission deadline of September 24, 2026.
