About this file
This document is a Notice of Intent to Award from the Michigan Department of Technology, Management & Budget (DTMB) Printing Services for Request for Quote (RFQ) #25*0361, specifically for printing 250,000 CHJ 549 Health Care Request Forms. The solicitation involved three bidders: Rotary Multiforms, Image Printing, and Walker Printery, who were evaluated on criteria including pricing, turnaround time, printed in Michigan status, and experience. The bid evaluation process focused on determining the best value vendor, with DTMB Printing Services reserving the right to perform the work in-house or select a higher-priced bidder if deemed more advantageous.
Rotary Multiforms was recommended as the awarded vendor with a bid of $14,016.75, offering a 15-working day turnaround and meeting most evaluation criteria. The total project quantity is 250,000 forms, with the print consultant Scott Barclay available for questions at 517-636-7744. Bidders were required to submit any protests electronically through Vendor Self Service by a specified deadline, with protests received even fractionally after the cutoff considered late. The document indicates a competitive bidding process emphasizing value, local economic considerations, and vendor capabilities beyond just lowest price.
