C. Proposals must be valid for a period of at least three hundred sixty-five (365) calendar days from the Proposal Submission Deadline. No Proposal may be withdrawn after the submission deadline.

D. All pages must be numbered and identified sequentially by section. Proposals must be tabbed and indexed in accordance with the information requested in the Section 6, Vendor

Responses Section. It is imperative that all Respondents comply, exactly and completely, with the instructions set forth herein. All responses to this solicitation shall be type written or word-processed (except where otherwise provided or noted), concise, straightforward, and must fully address each requirement and question. Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA referenced in any response if the material is included in the same section as additional information.

E. Proposals are not to be marked as confidential or proprietary. County may refuse to consider any Proposal so marked. All Proposals and supporting documents will be subject to the provisions of the California Public Records Act (California Code Government Code

7920 et seq.) (PRA) and will be disclosed or withheld in accordance therewith. County shall not be liable in any way for disclosure of any such records. Additionally, all Proposals shall become the property of County. Respondents should not request that certain information be treated as exempt, and statements in the Proposals should not be marked as confidential or proprietary.

F. Each Respondent shall exercise reasonable care and diligence to avoid submitting a

Proposal that could result in a conflict of interest if Respondent were to be selected. This obligation shall apply to Respondent, Respondent’s employees, agents, and relatives, sub-contractors, and third parties associated with accomplishing work and services in Proposal.

In the event Respondent has done work for County on this Project in the past or has reason to believe that a conflict of interest may exist for Respondent in regard to this Project, Respondent should consult with its legal counsel prior to responding to this solicitation.

Any Respondent who is found to have an actual conflict of interest may have its Proposal rejected on that ground. Respondent, its employees and/or consultants may be subject to the provisions of the California Political Reform Act of 1974 (the “Act”), which (1) requires such persons to disclose any financial interest that may be materially affected by services provided under this Contract, (2) prohibits such persons from making, or participating in making, decisions that could reasonably affect such interest; and (3) may require the filing a Statement of Economic Interest (Form 700). Proposal nonresponsive.

K. Where two or more Respondents desire to submit a single Proposal in response to this

RFP, they must do so on a prime/subcontractor basis rather than as a joint venture.

County intends to contract with a single organization that may subcontract with multiple organizations (team) but not with multiple organizations doing business as a joint venture.

1.5 EVALUATION PROCESS

Proposals deemed to meet all minimum submission requirements in Section 1.4 shall be evaluated, per Population, by an Evaluation Panel of job knowledge experts based on the responses to the requirements in this RFP. County may request clarifications, or otherwise verify the contents of the Proposal, including information about the Respondent, and proposed sub-contractors. County reserves the right to seek publicly available information about Respondents.

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA

Proposals will be scored based on the established evaluation criteria, and will be assigned points that measure the responsiveness to each identified criterion. The total number of points earned will be tallied for each Proposal, based upon Respondent(s) submitted written materials.

The scoring is based on evaluation of the Proposal as measured against the Proposal Scoring

Criteria, which includes County of Orange Procurement Preference Policy, in accordance with

Sections 1.8-103 and 4.3-112 of the 2024 Revised County Contract Policy Manual (CPM). A copy of the 2024 CPM can be found at https://cpo.ocgov.com/cpm. Proposals will be evaluated from the most advantageous to the least advantageous to the County.

Evaluation Process Stage

The evaluation process stage may be conducted in two phases. Phase 1 will be an evaluation of the written proposal based on the evaluation criteria for all Respondents and Phase 2 will be an evaluation of the interview/presentation for only the highest-rated Respondents in Phase 1 that are invited to participate in Phase 2. The Evaluation Panel has the right to determine whether to conduct Phase 2 and the number of Respondents who will participate in Phase 2 if conducted.

Please note that contract award for each Population will be made to the responsible Respondent whose proposal is most advantageous to County, taking into consideration the applicable evaluation criteria. Cost Proposal as an evaluation criterion does not require County to select

Respondent with the lowest cost proposal.

The Evaluation Panel will conduct the following evaluation process one time for each

Population. The scores for each Population is independent of, and will have no impact on, the scores for the other Population.

Phase 1:

In Phase 1, written proposals will be scored by the Evaluation Panel using the established weighted criteria set out in Section 5. Proposal Scoring Criteria, which have been assigned points before the submission of proposal that measure the responsiveness to each criterion, for each Population in the Proposal. This means a Respondent’s proposal may be evaluated up to two times (once for each Population) depending on the number of Populations submitted in the proposal.

The Evaluation Panel will evaluate the written materials solely on the quality of the responses to all questions and requirements of this RFP using the evaluation criteria. The total number of points earned for all criteria for the Population proposed will be tallied for each proposal, and the proposals will be rank-ordered based upon the points earned for that Population.

Phase 2:

If the Evaluation Panel decides to conduct Phase 2 for one or both Populations, then the selected highest-ranked Respondent(s) from Phase 1 for that Population will be invited to participate in

Phase 2. In Phase 2, Respondents must provide an oral interview/presentation on their proposed services. Respondents must be prepared to travel to County at their own expense to make their interview/presentation within five (5) calendar days after notification by the DPA. Respondents https://cpo.ocgov.com/cpm

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA must be prepared to discuss all aspects of their proposals in detail, including answering technical questions. Respondents are not allowed to alter or amend their proposal during the interview/presentation process.

Respondents may have up to two total scores (one for each Population) depending on the number of Populations submitted in the proposal. The total score for each Population shall be as follows:

Written Proposal Scoring: 800 points maximum

See "Proposal Scoring Criteria" section for categories and points.

Presentation/Interview Scoring: 100 points maximum

County reserves the right to conduct, or not to conduct, oral interviews/presentations with the highest-ranked Respondent(s). The decision whether to conduct oral interviews/presentations rests solely with County, and the decision of the DPA is final.

Please note that, if the Evaluation Panel does not conduct Phase 2 for a Population, the total score for all Respondents for that Population is the points awarded in Phase 1. If the Evaluation Panel does conduct Phase 2 for a Population, Respondents that County elects not to invite to participate in Phase 2 will not have a total score for that Population.

1.6 SELECTION/AWARD PROCEDURES

Upon the completion of the evaluation process, per Population, the Evaluation Panel will make a recommendation for an award to County’s DPA for award of a contract to a Respondent(s) for

Population A and for award of a contract to a Respondent(s) for Population B. Final award determination shall be subject to reference checks and past performance.

The Model Contract contained in this RFP is the contract proposed for execution. It may be modified to incorporate negotiated items and other pertinent terms and conditions set forth in this

RFP, including special conditions and requirements and those added by addendum, necessary attachments and/or exhibits, and to reflect Respondent’s Proposal and qualifications. Any additional exceptions to the terms and conditions made by any Respondent after submission of its

Proposal may result in its elimination from further consideration.

Exceptions to the terms and conditions of the proposed contract or the statements regarding

Respondent’s inability to comply with any of the provisions thereof are to be declared in the response to the RFP in VENDOR RESPONSES - Statement of Compliance.

Negotiations may or may not be conducted with the finalist(s); therefore, the Proposal submitted should contain Respondent(s) most favorable terms and conditions for each Population, since the selection and award may be made without further discussion or need for clarification.

County will enter into negotiations with the selected Respondent and award a contract upon completion of negotiation of fees and contract terms. The award requires approval(s) by County

Board of Supervisors.

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA

If a satisfactory contract cannot be negotiated in a timely manner, County, in its sole discretion, may terminate negotiations with the selected Respondent and begin negotiations with the next

Respondent.

1.7 RIGHTS RESERVED TO COUNTY

A. Make use of any information or ideas in the Proposals submitted.

Respondent responding to this solicitation;

I. Award its total requirements for each Population to one Respondent or to apportion those requirements among two or more Respondents and to award contracts to Respondent(s) on all or some of the Populations set forth in their proposals, as County may deem to be in its best interests;

J. Reject any Proposal if it is conditional, incomplete or deviates significantly from the services requested in this solicitation;

K. Request Best and Final Offer for each Population from any Respondent determined to be within the competitive range. Unless requested by County, late Best and Final Offers will

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA not be accepted by County for any reason. In addition, negotiations may or may not be conducted with Respondents. Therefore, Proposals submitted shall contain the

Respondent's most favorable terms and conditions, since the selection and award may be made without discussion with any Respondent.

L. Make multiple awards to Respondent(s) for services requested in this solicitation, at its sole discretion, to serve the best interests of the County.

M. Defer award of this contract for a period of 90 days after the solicitation opening. During this period of time, Respondent shall guarantee the prices quoted in their Proposals.

SECTION 3. SCOPE OF WORK

3.1 GENERAL INFORMATION:

Beginning July of 2026, all Behavioral Health Plans in California will be required to implement

High-Fidelity Wraparound (HFW) to fidelity under the Full Service Partnership (FSP) program. All FSPs for children and youth must align with the principles, values, and standards of the HFW model. HFW provides a comprehensive, holistic, evidence-based, child/youth and family-driven process for responding when children and youth experience significant mental health or behavioral challenges, including children and youth involved in multiple child/youth-serving systems.

HFW is a team-based and family-centered evidence-based practice that includes an “anything necessary” approach to care for children and youth with the most intensive mental health or behavioral challenges. The HFW model combines a team-based case management and facilitation approach with individualized and community-based mental health services and supports tailored to meet the individualized needs of the child/youth and family.

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA

HFW is intended to simplify the lives of the child/youth and family and increase their chances of resiliency and healing in the community by:

• Centering care and service planning within the context of a Child and Family Team

(CFT), guided by a trained professional facilitator, led by the voice of the child/youth and their caregiver(s).

• Developing a simple, individualized, time-limited plan of care with a structured, creative, and individualized set of strategies that are effective and relevant to the child/youth and their family.

• Empowering the child/youth, family, and natural supports to develop sustainable strategies that allow the child/youth to remain in school and in the community.

• Improving caregivers’ ability and confidence to identify and address the child's/youth’s needs.

3.2 TARGET POPULATIONS

Services will target two primary populations:

• Population A: Children, 0-17 years of age, who are living with a Serious Emotional

Disturbance (SED) and youth, 18-25 years of age, who are living with a Serious Mental

Disorder (SMD) per the definition in California Code, Welfare and Institutions Code -

(WIC) § 5600.3 | Strong consideration will be given to respondents who propose staffing and programming to serve culturally and linguistically underserved populations.

• Population B: Children, 0-17 years of age, who are living with SED and youth, 18-25 years of age, who are living with SMD, and are also systems-impacted (i.e., involved with Children Welfare, Probation, and/or Juvenile Collaborative Courts).

Programs must be able to serve all children and youth that meet the criteria for the population(s) they intend to serve. Respondents should be able to describe how they address the Whole Child

Model including medical, cultural and substance use needs. Programs must describe how they will meet the needs of LGBTQ children and youth.

SED is currently defined in the WIC as a mental disorder, as identified in the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders (currently the 5th Edition, Text

Revision), other than a primary substance use disorder or developmental disorder that results in behavior inappropriate to the child’s age according to expected developmental norms. Per the

WIC, members of this target population shall meet one or more of the following criteria:

A. As a result of the mental disorder, the child has substantial impairment in at least two of the following areas: self-care, school functioning, family relationships, or ability to function in the community; and either of the following occur:

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA

1. The child is at risk of removal from home or has already been removed from the home.

2. The mental disorder and impairments have been present for more than six months or are likely to continue for more than one year without treatment.

B. The child displays one of the following: psychotic features, risk of suicide, or risk of violence due to a mental disorder.

C. The child has been assessed pursuant to Article 2 (commencing with Section 56320) of

Chapter 4 of Part 30 of Division 4 of Title 2 of the Education Code and determined to have emotional disturbance, as defined in paragraph (4) of subdivision (c) of Section

300.8 of Title 34 of the Code of Federal Regulations.

SMD is currently defined in the WIC as a mental disorder that is severe in degree and persistent in duration, which may cause behavioral functioning that interferes substantially with the primary activities of daily living, and which may result in an inability to maintain stable adjustment and independent functioning without treatment, support, and rehabilitation for a long or indefinite period of time. Serious mental disorders include, but are not limited to, schizophrenia, bipolar disorder, post-traumatic stress disorder, as well as major affective disorders or other severely disabling mental disorders. This section does not exclude persons with a serious mental disorder and a diagnosis of a substance use disorder, developmental disability, or other physical or mental disorder. For specific criteria of this target population, as well as additional criteria related to outreach and treatment options, refer to California Welfare and Institutions Code (WIC) § 5600.3.

Services provided to a participant must also be medically necessary and clinically appropriate to address the participant’s presenting condition:

• Pursuant to Welfare and Institutions Code section 14184.402(a), for individuals 21 years of age or older, a service is “medically necessary” or a “medical necessity” when it is reasonable and necessary to protect life, to prevent significant illness or significant disability, or to alleviate severe pain as set forth in Welfare and Institutions Code section

14059.5.

• For individuals under 21 years of age, a service is “medically necessary” or a “medical necessity” if the service meets the standards set forth in Section 1396d(r)(5) of Title 42 of the United States Code. Services that sustain, support, improve, or make more tolerable a mental health condition are considered to ameliorate the mental health condition are thus medically necessary and covered as Early Periodic Screening, Diagnostic, and Treatment

(EPSDT) services.

3.3 SERVICES TO BE PROVIDED

The FSP HFW program will be conducted primarily in person and are expected to be field-based, including the following core group of Medi-Cal Specialty Mental Health Services (SMHS) HFW services:

• HFW Facilitation and Coordination – The initiation of, care planning for, and coordination of HFW for children, youth and their families. The HFW Facilitator in cooperation with the child/youth and caregiver works with a CFT to help the child/youth and family understand the CFT function and HFW process, identify immediate safety needs, develop and coordinate activities within an individualized plan of care, and establish coordination across involved child serving systems (e.g., behavioral health, child welfare, juvenile justice, schools, and courts) and community agencies.

• Needs Assessment and Documentation – Child/youth and family needs and strengths will be assessed using the Integrated Practice Child and Adolescent Needs and Strength

(IP-CANS) Tool. The HFW staff will administer the IP-CANS assessment and use it as a service planning tool with the CFT throughout HFW delivery to inform the individualized care plan and update it as necessary. In addition, DHCS requires that the

Pediatric Symptom Checklist 35 (PSC-35) is used in the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) program across the State of California.

• Individualized Care Planning, including Safety and Crisis Planning – An individualized written plan to identify what the child or youth and their family need with respect to care coordination, transition planning, and connections to natural supports based on their needs and preferences. The individualized plan of care includes a safety plan that identifies safety needs, risks factors for a crisis, and proactive strategies to avert and respond to crisis, while at the same time ensuring access to 24/7 crisis response as a backup.

o FSP Programs are expected to provide 24/7 crisis intervention and support 365 days a year and respond in-person to crises using program staff designated by the County to write WIC § 5150/5585 holds.

• Caregiver Peer Support – Direct support to caregivers provided by Caregiver Peer

Partners who are part of the CFT. Peer Support helps caregivers understand and navigate the HFW process, improving their understanding of the child's/youth’s needs, and responding to the child/youth in ways that improve their outcomes.

FSP programs will provide the full spectrum of Medi-Cal SMHS including therapy (individual, family, and group), intensive case management, psychiatry and medication support, as well as supportive services such as independent living and rehabilitation skills, housing and employment assistance. Additional services covered by Medi-Cal may include additional screening for

Health Care Agency - Children & Youth Full Service Partnership with High Fidelity Wraparound, 042-2934503-CA substance use disorders, assessment, Intensive-Home-Based Services, Peer Support, linkages to substance use disorder treatment as indicated, caregiver respite, and activities that may benefit children and youth beyond traditional therapies.

FSP programs will also provide outreach/in reach and engagement services to clients and their families in the home and community to address barriers and challenges relating to seeking and engaging in services. Ongoing engagement services shall be provided to maintain enrolled participants in their treatment plan inclusive of clinical and nonclinical services, including services to support maintaining housing.

Strong consideration will be given to respondents who propose staffing and programming to serve culturally and linguistically underserved populations for Population A.

Programs providing services to Population B system-involved children and youth should include a plan to implement Functional Family Therapy.

Programs may also choose to provide Community Defined Evidence-Based Practices (CDEPs) that have been selected in consultation with an agreement by the County. Providers shall be required to report data and outcomes on such services.

Given the intensity of children/youth and family needs, services will predominantly be conducted through in-person contacts in locations convenient for the family (e.g. home, school, community center). Telehealth may be used on a short-term basis if requested by the client and/or their support system and is determined to be clinically appropriate.

Programs proposing to serve transitional Age Youth, ages 18-25 in both populations, shall include a developmentally and clinically appropriate alternative FSP model: Full-Service

Partnership Intensive Case Management (FSP ICM) model.

FSP ICM requires a team-based approach with an identified team lead. FSP ICM teams are required to have a ratio of no more than 25 participants per FSP ICM team lead. In addition to the required team lead, FSP ICM teams should include a combination of partial and full-time providers such as prescribers, peer support specialists, registered nurses (RNs), and other qualified providers. Respondents should align the staffing model with the needs of the individuals receiving services.

FSP ICM teams are expected to provide as many contacts as needed to support an FSP participant’s recovery.
