About this file

This is a Change Order Notice issued by the Chief Procurement Office for General Services in Illinois, specifically addressing an amendment to the Metro Ethernet Service contract (Reference No. 25-448DOIT-TELEC-P-68707) with Fidium Enterprise Services, LLC. The change order removes two service locations from the master contract: DOT-R205 located at 1956 N Bolton Road in Freeport, Illinois, and DOT-R212 located at 13590 Garden Plain Road in Morrison, Illinois. The vendor advised the Department of Innovation and Technology (DoIT) that service delivery to these sites would require buildout costs not included in the original contract quote. The affected service lines at both locations were for 20 megabits per minute (20MB) Metro Ethernet service, with multiple line items associated with each site (lines 114-120 for DOT-R205 and lines 58-64 for DOT-R212).

The change order results in a contract value decrease of $51,600, reducing the total contract cost from $504,360.00 to $452,760.00. DoIT exercised its option to cancel service at these two sites rather than absorb the additional buildout costs. This amendment is being posted for the required 14-day notice period as mandated for change orders exceeding $25,000 or extending the timeline by 180 days or more, in accordance with Section 33E-9 of the Criminal Code and applicable procurement regulations.
