About this file

This document is a Change Order Notice from the Chief Procurement Office for General Services in Illinois regarding Contract P-31755 for an irradiator procured by the Emergency Management Agency (EMA). Due to manufacturing delays, the contract with Hopewell Design will be extended from June 30, 2022 to August 31, 2022. Approximately 70% of the services have been completed, but the final deliverable will not be finished until mid-August. The change order is necessary due to circumstances that were not reasonably foreseeable at the time the contract was signed, and the change is germane to the original contract.
