About this file

This document is a Change Order Notice issued by the Chief Procurement Office for General Services for the state of Illinois. It provides details regarding a change order for State Contract P-26780-JS, which extends the contract end date due to an unforeseen delay in the supply chain for parts needed for installation. The change order extends the contract expiration date from the original date to November 11, 2021 to accommodate the supply chain delay.

The Change Order Notice indicates that per state regulations, changes to the contract that increase the cost by more than $10,000 or extend the completion time by 30 days or more must be posted publicly for 14 days. The circumstances necessitating this change were not reasonably foreseeable at the time the original contract was signed, and the change is germane to the original contract.
