About this file
This document is a Change Order Notice issued by the Chief Procurement Office for General Services in Illinois. The change order is for the purchase of additional envelope stock needed to process rebate payments under the recently passed Illinois Family Relief Plan. The original contract with Midland Paper could not have anticipated these additional quantities, as the relief plan had not yet been passed at the time the contract was bid. The change order allows for an increase in the contract amount by up to $145,031.00 without requiring a formal change order, as the circumstances necessitating the change were not reasonably foreseeable.
The document cites the relevant sections of the Illinois Procurement Code and Administrative Rules that govern change orders. It provides the procurement title, reference number, and a detailed explanation for the need for the change order. This change order is in the best interest of the State of Illinois.

This Change Order Notice provides detail as to why the change order is needed and, pursuant to Section 33E-9 of the Criminal Code, that the circumstances said to necessitate the change in performance were not reasonably foreseeable at the time the contract was signed or the change is germane to the original contract as signed. Change orders are defined by Section 1-15.12 of the Procurement Code and Section 1.2067 of the CPO-GS' Administrative Rules.
