About this file
This is a grant application template for the State of Maryland's CFVPI-0004 program, which seeks to implement firearm violence prevention projects and programs using a public health approach to reduce homicide, suicide, and other forms of firearm violence throughout Maryland. Eligible applicants include non-profit organizations with 501(c)(3) status and government entities in Maryland. The application requires organizations to select one of three pillars: Prevention & Assessment (including protective order firearm surrender implementation, safe storage programs, violence reduction plans, or lethal means safety counseling), Intervention & Response (including community violence interruption, group violence intervention, cognitive behavioral approaches, hospital violence intervention, or crisis intervention teams), or Community Resilience and Healing (direct victim/survivor services). Applicants must submit a comprehensive project narrative addressing data-driven needs justification, detailed project descriptions including goals and objectives, organizational capacity documentation, staffing information, a work plan with proposed activities and deliverables, and a budget narrative. Required supporting documents include IRS non-profit designation letters (if applicable), letters of support, annual organizational budgets, and completed budget forms.
The grant period runs from July 1, 2026 through June 30, 2027. Applicants must provide detailed budget justifications organized by line item categories including salary, fringe benefits, contractual costs, travel, operating costs, supplies, and indirect costs (not to exceed 15%). The application requires data-driven justification using public health data, with encouragement to utilize the Maryland Firearm Violence Data Dashboard, and must include local demographic and geographic breakdowns where available. Applicants must demonstrate organizational subject matter expertise in firearm violence prevention, identify community partnerships with at least one letter of support, and certify compliance with American Disabilities Act requirements. The application template specifies word counts and formatting requirements for each narrative section and mandates submission of all documents in PDF format.
